Erythema Nodosum is a skin inflammatory condition. Skin is the delicate organ of the body. It protects us from various infections and also protects us from a too hot and too cold environment. It covers the entire external part of the body. Skin is the largest organ of the body and it consists of hair, sweat, and oil glands. It together makes up the integumentary system. So in this article, we are going to discuss a skin disease called erythema nodosum and its Ayurvedic management.


Erythema nodosum is an inflammatory condition of the skin. It is characterized by the inflammation of the fat cells of the skin. These fat cells are under the skin. There is a tender red nodule over the skin and also there are lumps that are usually seen over the shin bone (Tibia). This condition is a common acute nodular panniculitis that is characterized by the sudden onset of the firm, solid, deep nodules under the skin. These are painful on palpation and mainly present on extensor surfaces of the legs. This is a recurrent hypersensitivity reaction to a variety of antigens.

Erythema Nodosum


This condition is generally idiopathic, meaning the cause of this disease is not known. Instead of this, researchers found that the most common identifiable cause of this disease is streptococcal pharyngitis. Some of the other known causes are:


Some of the infections associated with this disease are:

  • Primary infection of tuberculosis
  • Histoplasma capsulatum
  • Yersinia
  • Epstein-Barr virus
  • Cat scratch disease

Autoimmune Disorders

This includes:

  • Sarcoidosis


The symptoms of this disease come in two phases i.e. Pre-eruptive phase and the Eruptive stage:

  1. Pre-eruptive Phase: The first sign of this disease is flu-like symptoms such as fever, cough, malaise, aching joints.
  2. Eruptive Phase: There is an appearance of 1-2 inch nodules under the skin surface. It is present usually on the shins. It can also appear on the arms and thighs. These nodules are hot and firm to touch. There is pain on these nodules on palpation.


  • Dermo hypodermitis
  • Nodular hypodermitis
  • Lobular hypodermitis


This disease is diagnosed clinically. A biopsy should be done to confirm this diagnosis. In this test, tissue is taken to examine under a microscope. Also, a sample of throat swab is taken to check for streptococcus infection.


This is a self-limiting disease and usually resolves itself in around 3-5 weeks. But there is a recurrence of this disease and is attributed to repeated infections. In this disease, symptomatic relief is the only treatment of choice and symptoms can be treated through bed rest, leg elevation, compressive bandages, wet dressing, and NSAIDs.

In Ayurveda, we can correlate this disease with Arsha. Our Acharya clearly mentioned that Arsha disease is a result of improper digestion. When the food is not digested and remains inside the gut then it is converted into Aama which is known as Endotoxins and this aama is the root cause of Arsha disease. The condition in which there is a severe pain like someone (enemy) stabbed at that point is known as Arsha or the mass that obstructs the passage of stool and causes pain is known as Arsha. The mass at gudavali (dentate line) is called Arsha. Arsha in the classical texts comes under the category of Mahagadas.

The term guda derived from Guda Kridayam tagu malotsarge means the portion of the body which is responsible for the excretion of mala (feces) and flatulence (apana vayu) is called as guda.


The exact cause of this disease is not known. But the doctors’ believe that this disease is caused by the following factors:

  • Irregular bowel habits (constipation or diarrhea)
  • Lack of exercise
  • Low fiber diet
  • Increased pressure in the lower rectum 
  • Having anal intercourse
  • Prolonged sitting


When the patient is doing the following things in their daily routine the Arsha disease is caused and those things are:

  • Eating excessive heavy, sweet, cold, stale food, improper diet
  • Over eating
  • Eating less food
  • Inaccurate food consumption
  • Drink too much ghee
  • Lack of exercise
  • Excessive coitus


These factors include:

  • Obesity
  • Prolonged sitting
  • Chronic cough
  • Pelvic floor dysfunction
  • Pregnancy


Piles are of two types:

  • Internal Piles: They lie inside the rectum and rarely cause discomfort. The patient has painless bleeding during bowel movement and if it progresses then may result in pain and irritation.
  • External Piles: They lie under the skin around the anus. They show features like itching or irritation in anal region, bleeding, pain.


Classification According To The Origin

There are two types:

  • Sahaja arsha (Congenital)
  • Janmottarkalaja arsha (Acquired)

Classification According To The Bleeding:

It is of two types:

  1. Shushka arsha
  2. Aadra arsha

Classification According To The Dosha

It is of five types:

  1. Vataj arsha
  2. Pittaj arsha
  3. Kaphaj arsha
  4. Sannipattaj arsha
  5. Raktaj arsha


  • Grade 1: There is small inflammation that usually lies inside the lining of the anus.
  • Grade 2: Their size is bigger than grade 1 but they also lie inside the lining of the anus. They get pushed out while defecating, but can return on their own.
  • Grade 3: This type is also known as prolapsed hemorrhoids. The patient may feel these masses hanging from the rectum, but they can easily be re-inserted with the help of fingers.
  • Grade 4: In this grade, these masses cannot be pushed back in and they need surgical treatment. Their size is also bigger and remains outside the anus.


In some cases these piles resolve on their own after a few days without giving signs and symptoms. But in most cases they may show the following symptoms:

  • Hard and painful lump around the anus
  • Itching or irritation around the anal region
  • Discomfort
  • Swelling around the anus
  • Bleeding
  • Feeling like that bowels are still full after defecation


The above quoted shloka means that the following symptoms are the Prodromal symptoms of the disease arsha and those are:

  • Indigestion
  • Weakness
  • Bloating
  • Excessive burping
  • Pain in feet
  • Constipation
  • IBS or IBD


The common symptoms of arsha are:

  • Presence of mass in the anus
  • Difficulty in passing stool
  • Indigestion
  • Bloating
  • Pain and burning sensation in anal region
  • Bad taste
  • Anemia
  • Flatulence


Complications are rare but if complication arises then those are:

  • Anemia
  • Strangulated hemorrhoids
  • Blood clot

Role of Parisechan (Sprinkling)6

This shloka states that pile mass is also sprinkled with the decoction of the leaves of Vasa (Adhatoda vasica), Arka (Calotropis procera), Eranda (Ricinus communis) and Bilva (Aegle marmelos). It will give relief from pain and swelling.

Role Of Avagahan(Sitz Bath)7

Acharaya Charak mentioned that if there is pain in the pile mass then it should be smeared with medicated oil and give sitz bath with the help of decoction prepared from the leaves of radish (Raphanus sativus), Amla (Emblica officinalis), harad (Terminalia chebula), bahera (Terminalia bellerica), Arka (Calotropis procera), Varuna (Crataeva nurvala), Agnimantha (Premna integrifolia), Shigru (Moringa oleifera) and Pashanbheda (Bergenia ligulata).

Role Of Abhyanga (Massage)8

Acharya Charak mentioned the role of massage therapy in pile mass. He said that the fat (charbi) variety of black snake, pig, camel, bats and cat should be applied on the pile mass and after then fumigation of piles should also done with the help of human hair, snake worm, cat’s skin, roots of Arka (Calotropis procera) and leaves of shami (Prosopis cineraria).

Ayurvedic Pralepa (Ointments) For Arsha9

This shloka states that we should apply the paste of herbs in the pile mass for pain and swelling. In pile mass we should apply the powder of haridra (Curcuma longa) mixed with latex of Snuhi (Euphorbia neriifolia) or we can also apply the paste prepared from the powders of seeds of Shirish (Albizia lebbeck), Kutha (Saussurea lappa), Pippali (Piper longum), Saindhav Lavan (Rock salt), Gurh (Jaggery), Latex of Arka (Calotropis procera), Amla (Emblica officinalis), harad (Terminalia chebula), bahera (Terminalia bellerica) on the pile mass.

Operative Procedures Include

Raktamokshan (Bloodletting)10

This is a minor OT procedure used in the treatment of piles and other vitiated pitta and blood disorders. This procedure is done when the blood is vitiated and does not come out on its own even after all these above-mentioned protocols. In such conditions Raktamokshan should be done. This is done with the help of Leech, sharp instruments or needle.

The Principle Treatment Of Choice For Raktaj Arsha (Bleeding Piles) Is11

This shloka means in bleeding piles if there is an association of vata dosha then one should give an unctuous and cold potency diet and if there is an association of Kapha dosha then one should give a rough and cold potency diet chart. If there is an association of Pitta and Kapha dosha in bleeding piles then detoxification therapy should be done. However, bleeding should not be stopped at the very first moment because it will cause more harmful effects on the body. This blood is vitiated and it is important not to prevent this.

Harmful Effects On The Body If Homeostasis Therapy Is Done At The Very Beginning12

Acharya Charak mentioned that if blood is arrested at the beginning then it may alter the functioning of Apana vayu and leads to many diseases like Bleeding disorders, fever, excessive thirst, indigestion, anorexia, jaundice, anemia, edema, skin disorders, obstruction in stool and urine, etc. therefore only after the elimination of vitiated blood from pile mass we should use hemostatic medicines to stop the bleeding.

This shloka states that to stop bleeding we should give the decoction of Kutaja (Holarrhena antidysenterica) mixed with the powder of Shunthi (Zingiber officinale) and also we can give the decoction of dadima (Punica granatum) mixed with the powder of shunthi (Zingiber officinale).

Punctate Inner Choroidopathy Treatment with Ayurveda

Punctate inner choroidopathy is a disease of Eye. The eye is the major organ in our body. This major organ helps in vision and also it is highly specialized for photoreception. Punctate Inner Choroidopathy


Punctate inner choroidopathy is an inflammatory condition that affects the choroid and retina of the eyes. This can lead to vision loss. This disease most commonly occurs in young women who are myopic. This term punctate inner choroidopathy was first used by Watzke, et al. in 19841. In this condition there is an inflammation in the retina and choroid part of the eyes. This disease is the average age of patients ranging from 16-40 years for this disease. The inflammation is limited to the back of the eye, meaning the posterior aspect and there is no inflammation in the front of the eye. This is the distinctive feature of Punctate inner choroidopathy.


The exact cause of PIC is not known. But researchers believe that this disease is thought to involve both genetic and environmental factors. It may also represent an autoimmune type of uveitis.


The symptoms of this disease are:

  • Appearance of gray-white or whitish yellow punctate at the areas of the inner choroid
  • Blind spot
  • Blurred vision
  • Photopsia

Cecal volvulus is a disease that is related to the intestines. Intestines are the muscular hollow tube which extends from the lower end of the stomach to the anus. These intestines are also called bowels. The food particles then pass through the stomach to the intestines in the form of Chyme and further digestion and absorption take place. Intestines are divided into two sections called the small intestine and the large intestine. Volvulus is a condition when portions of the intestine get tangled upon a mesenteric axis. This tangled condition can cause impairment of the blood supply which further can result in complete or partial obstruction of the lumen. In this article, we will discuss the term cecal volvulus which is a type of Volvulus and its Ayurvedic management.

Cecal volvulus is a rare condition in which a part of the intestine is entangled or a loop of the intestine twisted with itself and resulting in bowel obstruction. Cecal volvulus term is used to describe when the cecum (which is in between the small bowel and colon) twists on itself and detaches itself from the abdominal wall. This rare condition describes the torsion of the cecum which often results in obstruction. If this condition is unrecognized then it can result in intestinal perforation or fecal peritonitis. Usually, a person may mismatch this condition with normal stomach problems and often results in some complications.


In this condition, the terminal ileum and right colon are involved. It occurs when loose mesentery twists around an axis and it can occur when the cecum is untangled from a caudate to cephalad direction. Other possible causes are:

  • Hirschprung’s disease

  • Infections
  • Violent coughing fits
  • Enlargement of the colon

AHAs and their Significance in Skin Care

AHA are acid molecules that can be plant based or animal based. They are used as ingredients of beauty, hair and skin care products especially meant for damage repair and anti aging. They are used in the manufacture of toners, creams, masks, lotions, etc. and also as medicated ointments meant for use only by prescription. AHAs can be very effective if used correctly. This article explains what AHAs are, how they work and why they are used, and what natural foods and facial application products can be used in place of the chemical laden beauty products, so as to avoid the side effects.

AHA, Alpha Hydroxy Acids


AHA stands for Alpha Hydroxy Acids. The AHAs occur naturally in fruits and some animal products. AHAs are used extensively in beauty and skincare products because of their many beneficial effects on human skin, the most popular of which is the anti-aging property of AHAs. The following sources are currently used for the extraction of AHAs:

1. Citrus AHA Mask

Lemons, oranges, grapefruits, etc. are all rich in citric acid.  Not only the consumption of these citric fruits is beneficial for the body in many ways, the citric acid present in them is very effective in reducing the pigmentation i.e. dark colored spots and patches, and also the spots left after acne has occurred.

Mix well 2 tbsp milk with minimum 6 % fat with 1 tbsp of freshly squeezed juice of lemon, orange, tangerine or grapefruit. Leave the mixture for some time. Next, apply this paste liberally on the neck and the face, leaving the eyes. Keep it for half an hour, and then rinse with lukewarm water. Even if your skin is oily, avoid using soap or face wash. Pat dry and if required, use a moisturizer.

2. Sugar Honey AHA Mask

Raw, unprocessed sugar contains some of the glycolic acid content naturally occurring in sugarcane. Glycolic acid possesses excellent exfoliating properties, making it a common ingredient in scrubs and face washes. It is effective in removing the top layer of skin cells thus reducing the scars and marks that occurred because of the top layer of the skin being damaged. Crush 2 tbsp raw sugar in a pestle mortar. Try to use the most unprocessed and raw sugar you can get. Mix in it 2 tbsp naturally extracted honey and 1 tsp lemon juice. Apply the mix on your face and neck but avoid the eyes. Leave for 2-3 minutes only and wash off with lukewarm water.

3. Apple AHA Mask

Apples contain malic acid which is one of the major AHAs. The malic acid content is further boosted when vinegar is prepared with apples. It absorbs excess oil and mixture from the pores and deepest cavities in the skin, thus absorbing sebum, dead cells and dirt. You will need the most raw and unprocessed apple cider vinegar you can get. Take one tbsp of it and dab on your face using a cotton swab. Take care not to apply too close to the eyes. It may feel tingling or pinching on your skin as it dries up. When it dries completely, remove it by rubbing or with a wet wipe.

4. Tartaric AHA Mask

Tartaric acid is probably the softest on the skin therefore most suitable for sensitive skin. It is found in grapes, thus wine and champagne are excellent sources of tartaric AHAs. Mix 75 ml of champagne with fuller's earth and apply it on your face and neck. Leave it on for around 15-20 minutes and then rinse gently.

5. Lactic AHA Mask

Lactic acid is most commonly found in milk, yogurt, curds, etc. Some lactic acid is also found in grapefruits. People with dry skin can greatly benefit from lactic acid face masks since such masks can also balance excess dryness and replenish lost oils and nutrients from the skin. Lactic acid is also very mild and safe for application on even sensitive skin types. You can apply yogurt regularly on your face every three days. To prepare a natural face wash, mix 2 tsp fuller earth with 2 tbsp beaten curd or yogurt, and add in 2-3 tsp of grapefruit juice. Apply on the face and neck, and if you want to add a scrubbing effect to it add a tsp of sugar. Massage gently for two minutes and then wash off with lukewarm water. Use this face mask once a week.


Although AHAs are not extremely harmful for any skin type, every person's skin is different and may react with a substance in different ways. Some people may experience the following side effects upon their first use of AHA based products:

  • Minimal discomfort
  • Burning sensation
  • Itching and irritation
  • Warmth over the applied area
  • Eczema
  • Very small blisters

However, the side effects shall resolve on their own within a few days as the skin gets habitual of the product. You should apply ice on the affected area and avoid exposing skin to direct sun. You should not apply on skin that you have recently waxed or shaved, or skin that has new cuts or burns. Also, if you already suffer from eczema, psoriasis or any other inflammation of the skin, you should avoid using these products without proper guidance from a dermatologist. Pregnant and lactating women should also not use it.
