Isaacs' Syndrome (Neuromyotonia) Management with Ayurveda

ABSTRACT

The world is full of surprises and some surprises are good, but on the other hand some can be unpleasant. The most obvious example of unpleasant surprises are the diseases. Whenever our body responds to some unusual process going on inside and surprisingly out of nowhere you get diagnosed with a condition which you never heard about then that's quite an unpleasant moment. Some people have the mental strength to cope up with all of this but some may not have that stability. Isaacs Syndrome, Neuromyotonia

INTRODUCTION

Isaacs syndrome, as mentioned before, is also called neuromyotonia. It is a continuous muscle fibre activity syndrome in which there is a continuous twitching and contraction of muscles. The muscle stiffness due to recurrent twitching and contractions is progressive and results in diminishing of reflexes. The outlook of this condition can vary. The outlook generally depends on the severity of the disorder, the underlying cause, effectiveness of treatment and associated medical condition. But symptoms of isaacs syndrome or neuromyotonia generally worsen with time and significantly result in the restriction of movement. This restriction of movement results in the interference with daily activities. Modern science explains that this condition is not having any cure but many patients have successfully found ways to manage the symptoms in this condition.

That is all modern tells about Isaacs syndrome or neuromyotonia. But Ayurveda has a little different approach towards this condition. Ayurveda suggests that Isaacs syndrome or neuromyotonia is the result of excess vata dosha in the body. Vata dosha, which is responsible for any movement in the body whether voluntary and involuntary, when it gets aggravated gives very drastic effects or symptoms.

CAUSES OF ISSACS SYNDROME OR NEUROMYOTONIA

The condition is the result of an autoimmune process going on inside the body. This autoimmune process is targeting the peripheral nervous system. On the other hand, if there is absence of autoimmune process then the cause of Issacs syndrome or  neuromyotonia is not known by modern science.

On the other hand, as mentioned before Ayurveda suggests that Issacs syndrome or  neuromyotonia is the result of vata dosha aggravation in which ama production is simultaneously involved. Ama accumulation is the accumulation of endotoxins in the body that result in the blockage of  channels hence causing blockage to the pathways of  vata dosha or kinetic energy of the body.

SYMPTOMS OF ISSACS SYNDROME OR NEUROMYOTONIA

  • As mentioned in the key points, two main symptoms (twitching and contraction of muscles) of this condition are even present in sleep or under general anaesthesia. Most generally the muscles of arms and legs are affected but the other muscles of the body slowly get involved. 
  • The symptoms are as following:
  • Ataxia which is the trouble coordinating movement
  • There is a delay in the muscle relaxation
  • Excessive sweating along with changes in the heart rate is also noticed
  • Muscle cramps and muscle spasms are most commonly noticed. This results in weakness and progressive worsening of  the situation.
  • Myokymia, twitching and rippling or quivering of the muscles is also a very common symptom. This is generally explained in a horrifying way: Like having worms under your skin
  • One can also suffer from trouble while breathing and speaking if the muscles of the chest region are affected
  • Weight loss is also noticed

Now , as ayurveda suggests the involvement of vata dosha in this case. The twitching or loosing control even on the voluntary muscles as well as involuntary muscles depicts that vata dosha is aggravated in this case.

Vata aggravation has symptoms like samkocha which is contraction, sthambahna understood as rigidity, shosha is called as wasting pain, toda is piercing pain and vyatha which is aching pain. All these are the main symptoms in this case. So this justifies that here there is aggravation of vata dosha which can be associated with the accumulation of vata dosha. One typical symptom that correlates increased heart rate is mano vyaharsha (anxiety).

TYPES OF ISSACS SYNDROME OR NEUROMYOTONIA

Inherited Issacs Syndrome Or Neuromyotonia

This is passed down in families.

Acquired Issacs Syndrome Or Neuromyotonia

This is an autoimmune type of Issacs syndrome or neuromyotonia.

DIAGNOSIS OF ISSACS SYNDROME OR NEUROMYOTONIA

  1. The diagnosis procedure for this condition includes physical examination along with certain blood tests. Physical examination includes noticing the twitching of muscles especially on the face and hands.   
  2. Nerve conduction studies are done as well to understand  working of nerves and electromyograms are also done about the understanding of muscle working inside your body.
  3. CT scan (Computed tomography scan) along with MRI (Magnetic resonance imaging) is also done.

TREATMENT OF ISSACS SYNDROME OR NEUROMYOTONIA

The treatment of Issacs syndrome or neuromyotonia includes the following:

  • Generally a modern physician will prescribe you with a class of drugs called anticonvulsants. This gives relief in stiffness, muscle spasms and pain which is associated with isaacs syndrome.
  • Plasma exchange can also provide short term relief for the individuals who have acquired Issacs syndrome or  neuromyotonia.

AYURVEDIC ASPECT FOR ISSACS SYNDROME OR NEUROMYOTONIA

As mentioned before the condition is the result of aggravated vata dosha and aggravated vata dosha is having a very precise and peculiar management in the Ayurvedic science. The vata dosha is well managed by using various taila preparations in Ayurveda.

What is Mewing Technique? And How Does It Work?

Mewing is a technique in which the tongue is placed on the roof of the mouth in order to shape your jawline and appear more square. The tongue is a muscular organ which manipulates food for chewing and swallowing as part of the digestive system which is the primary organ of taste. The tongue's upper surface (dorsum) has certain taste buds which are housed in numerous lingual papillae. It is divided into two parts- an oral part at the front and a pharyngeal part at the back. The upper surface of the tongue is the dorsum and divided into the grooves into symmetrical halves by the median sulcus. There are 8 muscles of the human tongue which are classified as Intrinsic and Extrinsic. The jaw is an opposable articulated structure which is present at the entrance of the mouth which is mainly for grasping and manipulating food. INTRODUCTION

Mewing is the practice of repositioning the tongue inside the mouth with the goal of changing one jawline and appearance. AYURVEDIC ASPECT

In ayurveda, it is correlated with Khechari Mudra-

  • Khechara - to move
  • Mudra - posture

Khechari mudra can be well practiced and is the mudra of the Yoga Sadhana where both mind and tongue are mainly focused towards the sky. One must sit in Padmasana where both visions are fixed between eyebrows and by reversing the tongue. This is known as Khechari Mudra. There are two ways for performing-

  • Put the part of the frenum beneath the tongue and enable the tongue to move backwards and make an easy entry into the nasal cavity.
  • Frenum is not beneath the tongue and practice Talabya Kriya which enters the nasal cavity.

KHECHARI MUDRA STEPS

  • Sit in a comfortable position by using a chair or a blanket on the floor.
  • Close your eyes and curl the tongue back and in an upwards direction that should reach the soft spot on the roof of your mouth.
  • Apply gentle pressure on the point by using the tip of the tongue and remain in the position as long as you feel comfortable.
  • When you feel the pain, bring it to a normal position.
  • Relax and repeat the same process.

VARIOUS STAGES OF MUDRA

Stage 1 – soft palate of uvula-

  • It is done by extending the tongue and rolling it back as far as you can.
  • Reach to the hard palate and do fake swallowing to slide the tongue on the soft palate.
  • Do it 3-4 times until the tongue reaches the soft palate comfortably.
  • Push your tongue with a clean finger and slide your tongue in the mouth.
  • Try to reach your tongue at the bottom of your mouth where it touches Uvula.
  • Do 3-4 times to get the tongue at a comfortable point.

Stage 2 – Passing the Uvula

  • In the second stage, the Uvula tongue reaches farther than the Uvula region.
  • Open your glottis and make a bit of a blow inside the throat.
  • A strong burst of air is helpful in pushing the rolled tongue behind the uvula which enters into the nasopharynx.

Stage 3 – Slipping into Nasopharynx

  • When the tongue is moving behind the Uvula, it does not come to the normal position.
  • The tongue will slip and there will be a strong urge for throwing it out of the tongue.
  • Breathe slowly
  • The tongue will slip into the nasopharynx behind the soft palate and touch the bony structure known as the pituitary gland.

Stage 4 – Pressing the Pituitary To secrete Nectar

DNA is a group of molecules which is responsible for carrying and transmitting genetic materials. DNA is known as Deoxyribonucleic acid which is an organic compound which has a unique molecular structure, found in all prokaryotic and eukaryotic cells. It was first recognised and identified by Swiss Biologist Johannes Friedrich Miescher in 1869 while researching white blood cells. DNA comprises a sugar-phosphate backbone and the nucleotide base. It is a twisted ladder, has nucleic acid and all nucleic acids are made up of nucleotides each nucleotide has sugar, a phosphate group and nitrogen bases. The genomic DNA sequence has chromosomes which are found in each cell of the organism. Genes are the direct production of a protein with the assistance of enzymes and messenger molecules. Introduction

Genomics is the study of human genes and chromosomes. The genome typically consists of 23 pairs of chromosomes and 24,000 genes. Genes are transcription units which are capable of producing ribonucleic acid (RNA) transcript which can be translated into molecules of protein. Every organism has a basic set of chromosomes with a unique set of genes and DNA between them. It involves the sequence and analysis of genomes which uses high DNA sequencing and bioinformatics to obtain and analyze the function and structure of genomes. Organism Genome has cloned fragments which are amplified and replicated inside the living bacterial cell which reproduces quickly and generates many bacterial clones. A gene code is a polypeptide molecule with 3 nitrogenous base sequences for one amino acid.

Types Of Genomics

  1. STRUCTURAL GENOMICS - It is the structure of every protein which is encoded by the genome.
  2. FUNCTIONAL GENOMICS - It is to collect and use the data by describing gene and protein functions.
  3. COMPARATIVE GENOMICS - To compare the genomics between different species.
  4. MUTATION GENOMICS - It occurs in terms of mutation which occurs in a person's DNA and Genome.

Genomics And Its Correlation With Prakriti

Ayurveda is based on three fundamental things-

  1. Nature of a person's constitution
  2. Nature of the imbalance
  3. Nature of the medicine

Prakriti in Sanskrit prefix means 'pra' means original and 'Kruti' means creation. It is the inherent balance of three doshas which are the physiological forces of the body. The basic concept of Ayurveda is - 5 elements (panchabhuta) which constitute the physical universe including the human body, three doshas or constitutional types of the human body. Prakriti is a discrete phenotype which is determined on the basis of physical, psychological and behavioral traits. Prakriti and Genomics were hypothesized over a decade ago to be associated with single nucleotide polymorphisms in HLA-DRB1, CYP2C19, EGLN1 and Inflammatory and oxidative stress genes.

Ayurgenomics

It is a concept in Ayurveda such as Prakriti with modern genetics research. It usually correlates with a combination of three doshas, with an expression of specific genes and their physiological characteristics. The tridosha theory explains the three fundamental principles which govern the physiology of the individual. Vata doshas govern the transportation in the body to nerve impulses. Pitta doshas govern the process of digestion and metabolic pathway inside each cell formed from fire and water. Kapha dosha governs structure and cohesion in the body. Prakriti is the inherent constitution which is mainly established at the time of conception. It regulates the specific physiological processes, which is the constitutional phenotype which has a relation with genotype.

Genetic Basis Of Prakriti

  • In early 2005 study evaluated Prakriti and human leukocyte antigen (HLA) DRB1 type. It is found that HLA DRB1 is an allele in Vata Type and HLA DRB 1 (13) in the Kapha type.
  • In 2008, there is a correlation between biochemical and genome as in Pitta type there is overexpression of genes in the pathway of the immune system. In Vata males, there is overexpression in the regulation of cyclin-dependent protein kinase activity and enzyme activity. In Kapha, there is a regulation which involves fibrinolysis and upregulation of genes which involve ATP.
  • Kapha type has a higher level of triglycerides, total cholesterol and High LDL, Low HDL. In Pitta Type, there is a high amount of hemoglobin and red blood count. Prolactin is found higher in the Vata type.
  • In 2003, Kapha Prakriti had a higher level of Digoxin level which increases free radical production and increases tryptophan catabolites.
  • In 2014 research found that there is a correlation between Body mass index and Prakriti type. Kapha Prakriti has a higher BMI than Vata prakriti. The Vata-Kapha Prakriti has a higher risk of Diabetes Mellitus, hypertension and dyslipidemia.
  • In 2015, Studies shown patient with Irritable bowel syndrome is mostly seen in-
    1. IBS-C (Mainly seen in Vata type of patient)
    2. IBS-D (Mainly seen in Pitta dominant type of patient)

Epigenetics In Ayurveda

Epigenetics is a key component of Ayurveda which unites genotype and phenotypic variation. It describes the various factors (behavior, lifestyle, stress and diet, digestion) which usually influence the Deha (body) and Prakriti (psychological and physiological composition) related to phenotype, along with the genotype which is Janma (birth) Prakriti. Epigenetics is a critical component in Ayurveda and various changes such as Nutritional deficiencies, and environmental toxins produce epigenetic changes and result in illness.

The first food fad – "Try a calorie free diet"

Calories are the unit of energy. The amount of calories a person has is that much amount of energy. If the person is having more calories than required by the body, the body simply stores that in the form of fat. You know there is one more interesting fact that a packed food with a label of "FAT FREE" also has calories.

One more interesting fact is that high levels of calories will result in fat accumulation but low levels of calories also result in anaemia, infertility, bone loss, poor dental health along with decrease in thyroid function. Just not these many others can develop as well.

The second food fad – "Have a protein rich diet"

Proteins are required for the structural, functional and regulation of the body's tissues and organs. But what if you are having high amounts of proteins in your diet? Adequate might help you in building a body of your dreams but excess of course it will do the opposite. As excess of everything is bad. Excess amounts of protein have an impact on your hairs, it makes them rough and the hair strand loses its flexibility. It can deposit in various parts of your body like in kidneys forming kidney stones. Excess protein can disturb your digestion causing disturbance in the intestinal flora thus causing situations like recurrent diarrhoea or long term constipation, incomplete evacuation etc. It can also cause irritability and headaches as well.

The Third food fad – "A low carb diet"

What are carbs? They are carbohydrates. Carbohydrates are the primary source of energy and the human brain's preferred energy source. Carbohydrates are broken down by the body into glucose and this glucose is used by the body's cells, tissues and organs. If we start having a diet which is low in carbohydrates for a long time then ultimately your body will start breaking down fat into ketones to meet the requirement of energy. It is called ketosis. Ketosis can cause bad breath, headache, fatigue and weakness.

Cut the carbs, cut the carbs and cut the carbs. Your stomach may feel full but your body will starve.

The Fourth food fad – Ketogenic diet

Another diet for rapid weight loss is the ketogenic diet. Now this diet contains low carbohydrates but high fat. As we already know till now that both carbohydrates and fat are important for the body but in a balanced amount. But if we simply increase fat content in our diet and lower the carbohydrates then what do you think will happen? One can land with severely low blood sugar due to low carbohydrates and may develop High cholesterol problems due to high fat intake.

There are many other types of diet plans and yes those may also have some serious effects on the body, if used for a very long time. We must understand that it's not our clothing which can be changed whenever we like. Our diet patterns must be fixed because our body wants to have regular meals without too many conflicting tastes, at regular times of the day, year after year. So why are we opting for new diet plans?

If we discuss diet then yes diet is region specific. If fries, pizza, burger and other food articles are suitable to Person "A" who is an american by birth then it doesn't mean that it will be suitable for an Indian who has just shifted to America for work or living.

Sudden shift in your diets can cause a lot of digestion troubles and may hamper your metabolism. Some people shift from naturally available foods to frozen or canned food articles. This is also a very drastic shift in someone's diet. Why are we opting for preserved or canned food articles when we have resources? We can go to a supermarket, purchase some fresh vegetables and keep them in our fridge for 3-4 days or even a week.

Obviously preparing fresh things takes time but life's for when you cannot spend time preparing your own meal. Because the first step for taking care of your body starts not from dieting but it starts from your choices. What type of food you actually have, whether it's canned or fresh, freshly made or stored, ready to eat or self prepared, obviously these things matter if you're talking about eating healthy.

Now the person who is diet conscious also believes in labels. He or she will go to the market and look through every label. Some provide them with correct information, some labels confuse them and some convenience them.

Myths of labelling

As mentioned above some labels provide correct information, some labels are confusing and some are convincing. Your diet must not depend on labels as it's like putting your head inside a lion's mouth. Maybe you get it out on time, maybe not. Governments in various countries have introduced laws or have taken steps to standardise labelling in recent years yet there are still huge discrepancies in product labelling. Labels are the way of advertising so that the consumer can be convinced to buy their products, nothing more, nothing less. If you just want to have the nutritional information you may visit a library but reading labels is not going to help you. Manufacturers will never tell you that two medium size potatoes will provide you equal grams of protein as 30 grams of packed chicken. The main thing is what is the purpose of dieting and how you understand dieting. If you are dieting to make your body healthy then you must choose a diet which exactly makes you healthy not malnourished. Same applies if you want to lose weight as well or look pretty or want a glow on your face.

Have a diet that makes you lose weight and why to depend on diet alone, do exercise often. You will lose weight gradually. You want to look pretty and want that charm on your face for which you are spending so much on lotions and masks, then exercise, eat a healthy diet which is full of nutrients.

According to this we can divide dieting in two parts: healthy and unhealthy dieting or positive and negative dieting.

After this, what dangers exactly dieting without thinking brings. It can bring you your worst nightmare. Unhealthy or negative dieting can disrupt the metabolism to the point where it is hard to have any kind of decent health. This has become reality in western world and yes this negative dieting is far more widespread than you may imagine. We come across many people who have done so much damage to their bodies by too much dieting.

According to the Ayurvedic viewpoint, negative or unhealthy dieting causes damage to enzyme function. Ayurveda explains it as principles of agni. Agni is defined as the digestive fire and disruption in its working means invitation to many diseases. Other than digestion, agni is there in every cell of the body. Which means agni is responsible for the cellular function as well. If there is no balance of agni in the body then obviously disruption in the body's work will surely be noticed. And yes, unhealthy or negative dieting is capable of doing this.

Even eating things which are off season can also disrupt enzyme function or can cause disruption of agni. Like having mangoes in december!!

In addition, before opting for diet, one must understand the constitution of their body. Here, Ayurveda has the concept of prakriti. Vata prakriti, pitta prakriti and kapha prakriti. A peculiar type of diet can not work in all types of prakritis. For example, having raw fruits in large amounts, fruit juices or vegetable juices may be good for some people but for vata types this will never work.

As we were discussing the concept of agni, then disruption in its function results in ama or endotoxin accumulation. These endotoxins are capable of causing a variety of conditions starting from autoimmune conditions and may result in cancer as well. These days, alcohol and smoking are the secondary causes for building diseases in the human body but the diet has become the primary.

What are people doing these days? Cutting out nutrients from the food they have and have pills just to control their fat intake from the intestine. Yes, we are taking pills to block fat or nutrients and this is human empowering. You may control people around you, you may control situations around you, you can also control your own personal behaviour but sorry to say readers you cannot control your body. Body is like a temple, you have to worship it , take care of it and only after that you will see how much it can do for you. So, stop imaging to control your body and start working for it.

A mental disorder is a psychiatric disorder which causes significant changes and impairment of functioning. It usually involves significant disturbances in emotional behavior. According to DSM-IV, a mental disorder is a psychological syndrome associated with various symptoms such as distress and disability. It may be occasional and long-lasting (chronic). There are different types of mental health disorders such as Mood disorder, Anxiety disorder and Psychotic disorder. In this article, we are going to discuss its signs, symptoms and its ayurvedic aspect in detail.

INTRODUCTION

It is a mental disorder which is characterized by physical illness and injury and is usually known as somatic symptom disorder and somatization disorder. It causes stress and anxiety which is usually characterized by mental illness. Somatoform disorders are a set of psychological conditions. It was first described by Paul Briquet in 1859 and was known as Briquet's syndrome. Its symptoms are categorized into 4 categories such as neurological, cardiac, pain and gastrointestinal somatic symptoms. It is a severe form of somatisation which causes great distress, where anxiety and depression play an important role and some physical symptoms such as a Thumping heart.

TYPES OF SOMATOFORM DISORDER

Different Somatoform disorders are distinguished by various factors such as emotions, thoughts and multiple actions. There are seven types of somatoform conditions which include-

  • Somatization disorder
  • Conversion disorder
  • Pain disorder
  • Hypochondriasis
  • Body Dysmorphic disorder
  • Other specific somatic symptoms

1. Somatization Disorder

It varies from person to person and there is no physical condition. It persists for several years and involves pain, stomach issues and various neurological problems.

2. Conversion Disorder

It is an expression of psychology which involves loss of physical functions and has various neurological symptoms and is commonly known as Functional Neurological symptom disorder). It includes symptoms such as-

  • Weakness
  • Abnormal movements (tremors, seizures)
  • Blindness
  • Loss of sensation and numbness
  • Pseudoseizures

3. Pain Disorder

It is usually characterized by recurring pain in one or more parts of the body with no cause.

4. Hypochondriasis

It usually occurs when people are preoccupied with a concern that they have a serious issue. For example, it is believed that a minor headache is a sign of a brain tumor. The physical symptoms may be real or imagined.

5. Body Dysmorphic Disorder

It is a preoccupation with an imagined defect in a normal-appearing person.

6. Other Specific Somatic Disorder

It occurs which is less than six months and involves a condition known as Pseudocyesis (false belief). It usually includes false signs such as cessation of menstrual period, breast changes and fetal movement.

According to ayurveda, there are mainly three manas doshas- Sattva, Rajas and Tamas. These Sattva doshas especially suggest purity and quality of mind. Rajas Doshas represent the Dynamism and activity of the mind. Tamas Doshas represent Darkness. And this vitiation leads to all types of Mental disorders.

Ayurveda is an ancient science of life which proposes the concept of Doshas (Vata, Pitta and Kapha). The abnormalities in any doshas lead individuals to be diseased. Vata doshas are responsible for the function of all systems and maintain the whole body and its subtypes are Prana, Udana, Vyana, Samana and Apana. There is a unique concept in ayurveda which is explained well as Avarana. Avarana means covering and free-flowing Vata is obstructed with Pitta and Kapha, various tissues and excreta. In Avarana, Vata gets blocked by many things. The symptoms of Pitta and Kapha are not due to vitiated pitta and kapha but are due to pull by the force of Vata. Usually, there are 2 mechanisms of vitiation of Vata-

  1. Marga Avarana
    • Obstruction in the movements of Vata by tissues.
    • Obstruction of Vyana Vayu by Prana Vayu.
    • Withholding the natural urges of the body leads to an aggravation of Vata which obstructs its pathway.
  2. Dhatu Kshaya It is the depletion of the tissues which obstructs the passage and transportation.
    • Damage to various tissues
    • Deficient nutrition for tissues

This leads to Prakopa of Vata Doshas and enormous depletion of tissues. When Prana Vayu gets obstructed there are symptoms which are manifested as below-

CHIKITSA

In ayurveda, there are three types of chikitsa-

  1. Daivavyapasraya Chikitsa - It is the chikitsa which refers to performing measures like Mantra (incantation), Ausadhi (Wearing herbs) and Mani (Wearing gems). It is helpful in All types of Manasvikara in both Psychosis and Neurosis.
  2. Yuktivyapasraya Chikitsa (Rational therapy) - It refers to the use of Ahara (diet) and Aushadi (drugs). This Antahparimarjan Chikitsa, it is done with -
    • Samshodhana Chikitsa - Tridoshas are deranged with Raja and Tamas so purification therapy is suggested which maintains the homeostasis in the body. Various treatment procedures are - Vaman (emetic therapy), Virechan (Purgation), Nasya (Nasal Therapy) and Abhyanga (massage).
    • Sanshamana (Palliative Therapy) - When purification therapy is completed Shamana and Rasayan procedures are done to arrange the mind to normal. In this Rasaushadhi, Vati, Churna and Ghrit are given.
  3. Satvavajaya Chikitsa (Psychobehavioral therapy) - This therapy is to restrain the mind and in this Dincharya (daily regimen), Ritucharya (seasonal regimen), Sadvritta (code of conduct of life) and Achar Rasayan are followed.

  1. BRAHMI - Brahmi (Bacopa monnieri) is the famous gift of Ayurveda which is helpful in improving intelligence and memory. Has anti-inflammatory properties and is useful in all types of psychiatric disorders. Improves intelligence and digestion power. Reduces the blood sugar level and has an antioxidant property which improves cell and tissue health.
