ITP (Idiopathic Thrombocytopenic Purpura) – Ayurvedic Treatment, Diet & Home Remedies

Abstract

The main cause of almost every disease in Ayurveda is Agnimand (impaired digestive fire). The Agnimandya (digestive fire impairment) produces Ama (undigested toxins), which obstructs Srotas (micro-channels) and triggers autoimmune-like reactions, causing premature platelet destruction. Same goes with ITP. Immune Thrombocytopenic Purpura (ITP) is a blood disorder characterized by a dangerously low platelet count, making the body prone to excessive bleeding and bruising. Ayurveda sees it as Raktpitta (Bleeding disorder) in which there is loss of blood from external orifices of the body and specifically Tiryak Raktpitta in which blood oozes out from the hair follicles in the body.

Immune Thrombocytopenic Purpura (ITP)

Introduction

Immune Thrombocytopenic Purpura (ITP) is an autoimmune disorder in which the body’s immune system mistakenly attacks its own platelets, the vital blood cells responsible for clotting. As seen as Rakta Dhatu Kshaya (depletion of the blood tissue) and Rakta Pitta (bleeding disorders) caused by Pitta aggravation along with Vata vitiation. The aggravated Pitta vitiates Rakta Dhatu (Blood), leading to qualitative and quantitative abnormalities in blood, while Vata contributes to abnormal movement and destruction of platelets. This leads to a low platelet count, increasing the risk of easy bruising, spontaneous bleeding, and prolonged bleeding from minor injuries.

Although relatively rare, ITP has a significant impact on quality of life due to its bleeding complications. The pathogenesis of ITP involves immune system dysregulation where autoantibodies, primarily immunoglobulin G (IgG), attach to platelet surfaces, marking them for destruction by the spleen. Additionally, these antibodies can impair platelet production in the bone marrow by affecting megakaryocytes. Cytotoxic T cells further contribute by directly targeting both platelets and megakaryocytes. This combination of increased platelet destruction and reduced production reflects the complex nature of ITP, highlighting the importance of therapies that modulate immune activity while supporting the body’s natural platelet production and overall health.

Causes Of ITP

  • 1. Autoimmune response – The body's immune system mistakenly attacks its own platelets.
  • 2. Viral infections: Hepatitis B & C, HIV
  • 3. Bacterial infections: Helicobacter pylori (H. pylori) infection
  • 4. Vaccinations: Measles-mumps-rubella (MMR) vaccine, Influenza vaccine (rarely)

  • 6. Certain Medications
  • 7. Chronic lymphocytic leukemia (CLL) or lymphomas

Symptoms Of ITP

  • 1. Easy bruising (ecchymosis) without injury

Ayurvedic Overview Of ITP

In Ayurveda, it is primarily correlated with Rakta Pitta, a disorder where aggravated Pitta Dosha vitiates Rakta Dhatu, causing spontaneous bleeding. Mostly it is associated with Tiryak Raktpitta as mentioned by Acharya Charak. It is mainly derived from the Āma—the toxic, improperly digested metabolic residue—plays a significant role in the development and aggravation of Idiopathic Thrombocytopenic Purpura (ITP).

When Agni (digestive fire) is weakened (Agnimandya), incomplete digestion produces Āma, which enters the Rasa Dhatu and obstructs Rakta Vaha Srotas (blood-carrying microchannels). This blockage interferes with the nourishment (Poshana) of Rakta Dhatu, leading to Rakta Dhatu Kshaya (depletion) and impaired platelet production. Moreover, Āma acts as an antigen-like foreign substance, provoking an Ama-Visha state where the immune system (Ojas) misidentifies the body’s own platelets as harmful, triggering their premature destruction—similar to an autoimmune reaction. The resulting Pitta aggravation causes Rakta Pitta Lakshana (bleeding tendencies), while Vata disturbance accelerates the breakdown and loss of platelets. Clinically, this manifests as petechiae, mucosal hemorrhages, prolonged bleeding, and weakness, which correlate with Pittaja Rakta Pitta complicated by Āma.

Nidana (Causative Factors)

Excess intake of Amla (sour), Katu (pungent), Ushna (hot) foods, irregular diet, excessive heat exposure, emotional stress, seasonal Pitta aggravation, chronic indigestion (Agnimandya)

Dosha Dushti

Pitta ↑ (Ushna, Tikshna, Drava, Sara Gunas aggravated) → vitiation of Rakta Dhatu.
Vata ↑ (due to Dhatu Kshaya) → abnormal circulation & platelet destruction.

बहुश्लेष्मवाते तु शरीरे श्लेष्मवातसंसर्गाद्द्वावपि मार्गौ प्रतिपद्यते, तौ मार्गौ प्रतिपद्यमानं सर्वेभ्य एव यथोक्तेभ्यः खेभ्यः प्रच्यवते शरीरस्य||च.नि.२/८||

This describes how the combined vitiation of Vata and Kapha spreads pathology across multiple systems, affecting the entire network of Srotas, which can aggravate diseases with multi-system involvement — much like autoimmune disorders where multiple channels and tissues get affected. Acharya Charaka in Charaka Samhita mentions that vitiated Pitta and Rakta lead to bleeding disorders. And it is called as “Antaki Gati” by Acharya Charaka. This can be managed using blood-purifying herbs and therapies that strengthen Rasadi Dhatus. Moreover, Acharya has mentioned Vasa Ghrit, Shatavari Ghrit, Priyanguadi Peya, Atrushakadi Kwath, Khajooradi Tarpan, Laja Tarpan as its main formulations.

Acharya Sushruta in Sushruta Samhita describes Raktapitta management through Shodhana (detoxification), Shamana (pacifying) treatment, and Rasayana therapy to maintain long-term blood health and balance the Doshas.

Ayurvedic Remedies for ITP - Immune Thrombocytopenic Purpura

Idiopathic Thrombocytopenic Purpura or Immune Thrombocytopenic Purpura (ITP) is a syndrome in which our immune system gets activated against our own body cells thereby resulting in decreased count of platelets. As a result of which reddish or purple spots, bruises, internal or external bleeding etc can occur in the body depending on the severity of the condition.

AYURVEDIC CONCEPT OF ITP

In Ayurveda, ITP comes under the context of 'Raktapitta' (bleeding disorders). Here, aggravation of pitta dosha takes place in the body which in turn vitiates the rakta dhatu (blood). This vitiation mainly commences in the spleen and liver, as these are considered to be the main sites of rakta (blood) and progresses to other parts of the body. Although modern science considers it to be a disease of unknown origin, Ayurveda mentions the excess usage of ushna (hot), theekshana (sharp), amla (sour), katu(pungent) and vidahi (which causes burning sensation) food items, over exposure to heat and sun etc as the main underlying causes for ITP.

AYURVEDIC TREATMENT

In the treatment of ITP, Ayurveda mainly focuses on pitta shamana (normalising pitta), without disturbing the other doshas. Strength of the patient, severity of disease and abstaining from the etiological factors are also given prime importance. Either ‘Langhana’ (fasting) or ‘Tarpana’ (nourishing therapy) is administered by considering the involvement of ‘amadosha’ (toxic waste). Elimination therapies like ‘vamana’ (medicated emesis) or ‘virechana’ are advocated in extremely aggravated dosha conditions keeping in view of the strength of the patient. Strict diet and regimen has to be followed post these therapies.

For drinking purposes, water boiled and cooled with drugs like chandana (Santalum album), usheera (Vetiveria zizanioides), Musta (Cyperus rotundus), Parpataka (Fumaria officinalis) etc is generally given. Otherwise simply boiled and cooled water can also be used. Water is given either in small quantities or till the limit of satisfaction. Since madhura (sweet), thiktha(bitter) and kasaya (astringent) tastes reduce pitta dosha, these can be predominantly used by ITP patients. Commonly available items like Aloe vera, wheat grass, Papaya leaves, pomegranate, gooseberry etc can be used judiciously in this condition as it helps in increasing the platelet count. Despite being sour in nature gooseberry is indicated in ITP, as it is having sweet taste after digestion which is beneficial. Sheeta Virya (cold potency) drugs like Arjuna (Terminalia arjuna), Durva (Cynodon dactylon), chandana (Santalum album), Ashoka (Saraca asoca) etc are much useful in pitta imbalance disorders like ITP.
