Treatment of PID (Pelvic Inflammatory Disease) in Ayurveda with Herbal Remedies

ABSTRACT

Pelvic Inflammatory disease (PID) is an infection of the Women’s reproductive organs which is commonly caused by Sexually transmitted bacteria which gets spread from the vagina to the uterus, fallopian tubes and ovaries. The female reproductive system consists of Internal and external organs which create hormones and are further responsible for ovulation, menstruation and sexual activities. External genitalia protects the internal organs which allows the entry of sperm into the vagina. Menarche is the female’s first menstrual cycle and the normal menstrual cycle is divided into follicular and luteal phases and is more variable in its time course. In this article, we are going to discuss the causes and symptoms of pelvic inflammatory disease with its Ayurvedic aspect in detail.

Pelvic Inflammatory Disease

INTRODUCTION

Untreated Sexually Transmitted Disease (STDs) leads to Pelvic Inflammatory Disease which is a serious condition caused by Chlamydia and Gonorrhea. Sometimes it leads to serious complications, as the bacteria first enter the vagina and cause infection, with the passage of time it moves to pelvic organs. It can be a life-threatening complication when it gets spread to the bloodstream. There is Pain in the lower abdomen and there is unusual discharge from the vagina. About 770,000 women in the United States are diagnosed with Pelvic Inflammatory disease each year. The complications which include Short-term complications are- Tubo-ovarian and pelvic abscess, whereas Long term Complications include Ectopic pregnancy, Infertility and chronic pain in the pelvis.

PATHOPHYSIOLOGY

  • Infection of the upper female genital tract leads to inflammatory damage which results in Adhesions and total obstruction of the Fallopian Tube.
  • This adhesion leads to the complete loss of ciliated epithelial cells with fallopian tubes resulting in impaired transport of the ovum so there is an increased risk of infertility and ectopic pregnancy.
  • Sometimes this adhesion leads to chronic pelvic pain.

CAUSES OF PELVIC INFLAMMATORY DISEASE

  • Chlamydia trachomatis and Neisseria gonorrhoeae are the main causes of PID (Pelvic inflammatory disease) due to unprotected sex.
  • It can occur due to Untreated infections such as Bacterial vaginosis.
  • Another Sexually transmitted infection known as Mycoplasma genitalium increases the risk of PID (Pelvic inflammatory disease)
  • Other causes such as Abortion, Childbirth and various pelvic procedures.

COMPLICATIONS

Untreated Pelvic inflammatory disorder leads to the formation of Scars and pockets of infections to develop in the reproductive system, which permanently damages the reproductive organs.

  • Ectopic pregnancy- It is due to the formation of scar tissue in the fallopian tubes, which prevents the fertilized egg from getting implanted. It is life-threatening and requires medical attention.

  • Tubo-Ovarian Abscess- It is the collection of the pus in the reproductive tract. It is a life-threatening infection.

AYURVEDIC ASPECT

As per Ayurveda, it is due to an imbalance of tridoshas which is responsible for all health and health-related complications. According to Ayurveda Pelvic inflammatory disease is due to vitiation of Pitta and Vata doshas. As Vata doshas cause pain and difficulty during intercourse. Pitta vitiations are related to inflammation and a burning sensation. Due to the vitiation of Kapha doshas, there is a build-up of Ama (toxins) which gets mixed with blood and further reaches the reproductive system. Mainly Apana Vata is responsible for the management and the proper and undisturbed functions of pelvic organs.

CHIKITSA

The main aim of the treatment of pelvic inflammatory disease is to reduce pain and swelling and to control the aggravation of Pitta and Vata doshas.

1. Role of Panchakarma

It is helpful in expelling the vitiated doshas by shodhana (cleansing or expelling of doshas).

  • Virechana- It is the best way to expel the morbid pitta to control the inflammation. When the vitiated Vata and Pitta are under control then the pain and inflammation subside. Virechana is administered after the Snehana and Swedana which totally expels the morbid doshas. 
  • Basti- Basti is medicated enema which expels out the morbid Vata doshas and supports- strengthens the tissues. All 3 types of Basti are beneficial: Kashaya Basti- decoction enemas, Sneha Basti- Enema with medicated oils and ghee and Uttara Basti- Enema are given into the uterus. Matra Basti- Small doses of oil and ghee enema are given which will strengthen the organs along with pacifying the morbid Vata and Pitta.

2. Shotha Chikitsa

It is the Shamana chikitsa which pacifies the vitiated doshas.

  • After proper Aamapaachana, Agnideepana and Santarpana are done.
  • Parisheka with Triphala Kwath
  • Yoni Pichu (tampons with medicated oils)

3. Rasayana

It is used after Shodhana therapy which boosts the immunity in the body and prevents the progression of the disease.

1. Lodhra

Lodhra (Symplocos racemosa) is helpful in all female reproductive systems and useful in pacifying the pitta doshas. Improves the health of women and is useful in the detoxification of the body. 2. Ashoka

In recent years a number of viral conditions are spreading at an exponential rate. Like CoronaVirus affected a number of people worldwide, many lost their lives and others suffered major economic losses. Some of the people were hit severely by the repetitive corona waves. Now another virus namely Monkeypox is trending these days and people are scared of the virus. It could again affect many people worldwide. Monkeypox virus is an infectious condition caused by monkeypox virus. It spreads through a number of routes which makes it an issue of concern and manifests with various symptoms. In India, there have been eight cases reported so far. INTRODUCTION

Monkeypox is an infectious condition caused by a virus. It usually affects animals and humans. Pseudomonas Aeruginosa is member of the Gamma Proteobacteria class of Bacteria. It is a Gram-negative, aerobic rod belonging to the bacterial family Pseudomonadaceae. It is free living bacterium mostly found is soil and water. It may be found on the surface of plants and animals.

It is one of the most common opportunistic pathogen in healthcare associated infections like nosocomial urinary tract infections, nosocomial pneumonia, surgical site infections and nosocomial bloodstream infections. It is the most common cause of gram-negative infection especially in patients with compromised host defense mechanisms. Patients with cancer and those suffering from severe burns and cyst fibrosis are at high risk of getting infected. Pseudomonal infections can be life threatening.

SYMPTOMS OF PSEUDOMONAS AERUGINOSA

The patient has fever as the most common symptom Pseudomonas Aeruginosa.

Pneumonia happens in case of respiratory tract infection. The patient experiences fever, chills, difficulty in breathing, cough with and without sputum.

Bacteremia in case the infection reaches the blood stream. The patient experiences fever, chills, fatigue, muscles and joint pain, low blood pressure.

The fever and diarrhea may occur due to infection in GI tract.

Media and otitis externa when ear is involved. The patient complains of ear ache.

In case of eyes infection the patient may experience inflammation, pus, swelling, redness, pain and impaired vision.

DIAGNOSIS OF PSEUDOMONAS AERUGINOSA

Diagnosis of Pseudomonas Aeruginosa depends on culturing the organism from the site of infection: blood, skin lesions drainage fluid, urine, eye etc.

There could be greenish pus and fruity smell in case of localized infections.

Other tests like chest radiography, brain CT or MRI, Triple phase bone scanning, renal ultrasonography, echocardiography are performed depending on the individual's case.

Dosage: The recommended dosage for Navkarshik Churna is 1 table spoon two times a day.

To prevent the spread of infection, an approach targeting environmental contamination, good use of antimicrobials, and minimal use of invasive devices is needed. It is advised to perform hand wash hygiene, using hand soaps from non resuable containers and not using soap bars. One needs to cut finger nails short to minimize the spread of infection.

In modern medicine, the treatment option is a combination of antipseudomonal bata lactem and aminoglycosides. Surgical may be required for abscess drainage, diabetic foot, malignant otitis etc.

As a home remedy, intake of honey and applying honey over the infected areas is recommended. It is a natural antiseptic in the management of wound infections.

Ayurvedic Treatment for Intussusception - Cause, Symptoms, Diagnose & Herbal Remedies

Intussusception is a form of bowel obstruction which is a serious condition wherein one segment of the intestine telescopes or slides inside of another leading to blockage or obstruction of intestine. It blocks the food and fluid from passing through. It mainly occurs at the junction of the small and large intestines. The obstruction often causes swelling and inflammation which can lead to intestinal injury. It cuts off the blood supply to the affected part of the intestine thus leading to a tear in the bowel, infection and death of the bowel tissue.

Intussusception is one of the most common abdominal emergency conditions affecting children less than 3 years of age. It is very common in babies and more common in boys than girls. In children, an X-ray procedure is able to push back the intestine in its position. In adults, intussusception is mainly due to a tumor or polyps which often requires surgery.

Intussusception may occur in colon, small bowel or between the small bowel and colon. It occurs most frequently during the fall and winter months than rest of the year.

According to Ayurveda, obstruction means upward or backward or reverse movement of vata disorder.

CAUSE OF INTUSSUSCEPTION

The cause of intussusception is unknown. Some, viral or bacterial infection of intestine might lead to intussusception. In some children, it is caused by a condition that the child is born with such as a polyp or diverticulum. It could be due to enlarged lymph nodes or blood vessel abnormality in the intestines.

Weight loss surgery or a surgery of intestinal tract is another cause of intussusception. One may get the problem due to inflammation caused by Crohn’s disease.

Siblings of someone who has the problem are at more risk of getting it.

SYMPTOMS OF INTUSSUSCEPTION

The most common symptoms of intussusception are:

  • Severe, crampy abdominal pain lasting for 15 minutes which alters with periods without pain for 30 minutes. This is termed as clocking pain. Small children often bring their knees to the chest when they are feeling the pain. The children are seen crying due to pain due to compression of blood vessels and nerves.
  • Nausea, vomiting, fever and diarrhea. Vomiting is not due to eating. It may be bilious or yellow green colored bitter tasting vomit.
  • Rectal bleeding mixed with mucus. This is called currant jelly stool. It is a sign of lost blood supply in the bowel.
  • The patient feels weak and tired.
  • A lump in the abdomen can be felt. There is abdominal swelling
  • Flu like symptoms are experienced by the patient.
  • A weak and rapid pulse rate along with abnormal breathing, There is drop in blood pressure and the child may appear drowsy.
  • Skin may look pale or gray.
  • Anxiety and agitation.

DIAGNOSE OF INTUSSUSCEPTION

Early detection of intussusception is very important as it is a serious condition which is life threatening. Early diagnoses helps to prevent damage of the intestine.

A doctor can diagnose the problem by checking the symptoms of the patient.  A stethoscope can be used to hear the diminished bowel sounds. Abdominal lump can be felt by the doctor.

Abdominal X-rays and ultrasound are done to confirm intestinal obstruction. A barium, water soluble contrast or air enema is performed.

TREATMENT OF INTUSSUSCEPTION

The treatment is determined depending on the extent of the problem and the current health of the patient.

Ayurvedic Treatment for Pangastritis - Symptoms, Causes, Home Remedies, Diet & Lifestyle

Pangastritis is defined as the inflammation of entire lining of the stomach. The inflammation of gastritis is most often seen as the result of infection with the same bacterium that causes stomach ulcers. Pangastritis can occur suddenly known as acute gastritis or appear slowly over time known as chronic gastritis.

When we eat food, it goes down through the food pipe and it is stored in stomach for about 1.5-2 hours. In the stomach, food undergoes disintegration into small particles. HCl (Hydrochloric Acid) kills the micro-organisms present in the food. The small portion of the food is absorbed. After 1.5-2 hours, the food is pushed down to intestine for further digestion.

If the inner lining of the stomach is irritated or damaged or inflamed, this condition is known as gastritis. The stomach lining can become inflamed due to certain reasons like heavy alcohol consumption, eating spicy and junk food, stress etc.

WHAT ARE THE SYMPTOMS OF PANGASTRITIS?

  • Nausea
  • Vomiting
  • Burning ache or pain in the upper abdomen
  • Feeling of fullness in the upper abdomen after eating.

WHAT ARE THE CAUSES OF PANGASTRITIS?

  • In erosive gastritis drugs, alcohol, acute stress and infection can precipitate the condition.
  • In most non-erosive gastritis conditions, H.pylori often found in people who suffer from no significant symptoms of gastritis. The symptoms appear when the gut becomes a conducive environment for H.pylori to colonise. According to Ayurvedic concept, the inbuild of toxins and impurities, blockage of proper circulation in the gut creates ground for mucosal irritation from both the toxins and H.pylori growth.

According to Ayurveda, root causes of Pangastritis are poor eating habits, taking incompatible food, intake of spicy and fried foods. Due to these foods, food is not digested properly and it provokes the excess of acidic secretion and thus results in gastric irritation.

  3. Crush cumin seeds into a fine powder. Add half a teaspoon of cumin seeds to 1 litre of water and boil the water for 2 minutes and then filter it. Drink this water regularly in place of normal water. But, prepare fresh water daily. This is a natural remedy for pangastritis.
