ITP (Idiopathic Thrombocytopenic Purpura) – Ayurvedic Treatment, Diet & Home Remedies

Abstract

The main cause of almost every disease in Ayurveda is Agnimand (impaired digestive fire). The Agnimandya (digestive fire impairment) produces Ama (undigested toxins), which obstructs Srotas (micro-channels) and triggers autoimmune-like reactions, causing premature platelet destruction. Same goes with ITP. Immune Thrombocytopenic Purpura (ITP) is a blood disorder characterized by a dangerously low platelet count, making the body prone to excessive bleeding and bruising. Ayurveda sees it as Raktpitta (Bleeding disorder) in which there is loss of blood from external orifices of the body and specifically Tiryak Raktpitta in which blood oozes out from the hair follicles in the body.

Immune Thrombocytopenic Purpura (ITP)

Introduction

Immune Thrombocytopenic Purpura (ITP) is an autoimmune disorder in which the body’s immune system mistakenly attacks its own platelets, the vital blood cells responsible for clotting. As seen as Rakta Dhatu Kshaya (depletion of the blood tissue) and Rakta Pitta (bleeding disorders) caused by Pitta aggravation along with Vata vitiation. The aggravated Pitta vitiates Rakta Dhatu (Blood), leading to qualitative and quantitative abnormalities in blood, while Vata contributes to abnormal movement and destruction of platelets. This leads to a low platelet count, increasing the risk of easy bruising, spontaneous bleeding, and prolonged bleeding from minor injuries.

Although relatively rare, ITP has a significant impact on quality of life due to its bleeding complications. The pathogenesis of ITP involves immune system dysregulation where autoantibodies, primarily immunoglobulin G (IgG), attach to platelet surfaces, marking them for destruction by the spleen. Additionally, these antibodies can impair platelet production in the bone marrow by affecting megakaryocytes. Cytotoxic T cells further contribute by directly targeting both platelets and megakaryocytes. This combination of increased platelet destruction and reduced production reflects the complex nature of ITP, highlighting the importance of therapies that modulate immune activity while supporting the body’s natural platelet production and overall health.

Ayurvedic Overview Of ITP

In Ayurveda, it is primarily correlated with Rakta Pitta, a disorder where aggravated Pitta Dosha vitiates Rakta Dhatu, causing spontaneous bleeding. Mostly it is associated with Tiryak Raktpitta as mentioned by Acharya Charak. It is mainly derived from the Āma—the toxic, improperly digested metabolic residue—plays a significant role in the development and aggravation of Idiopathic Thrombocytopenic Purpura (ITP).

When Agni (digestive fire) is weakened (Agnimandya), incomplete digestion produces Āma, which enters the Rasa Dhatu and obstructs Rakta Vaha Srotas (blood-carrying microchannels). This blockage interferes with the nourishment (Poshana) of Rakta Dhatu, leading to Rakta Dhatu Kshaya (depletion) and impaired platelet production. Moreover, Āma acts as an antigen-like foreign substance, provoking an Ama-Visha state where the immune system (Ojas) misidentifies the body’s own platelets as harmful, triggering their premature destruction—similar to an autoimmune reaction. The resulting Pitta aggravation causes Rakta Pitta Lakshana (bleeding tendencies), while Vata disturbance accelerates the breakdown and loss of platelets. Clinically, this manifests as petechiae, mucosal hemorrhages, prolonged bleeding, and weakness, which correlate with Pittaja Rakta Pitta complicated by Āma.

METHOD OF PREPARATION

All the ingredients—Shuddha Hingula (Red sulphide of mercury), Nag Bhasma (Lead calx), Vanga Bhasma (Tin calx), various other aromatic and digestive herbal ingredients, Draksha Kalka (grape paste), Devdaru Churna (cedarwood), and Loha Bhasma (Iron calx)—are first finely powdered and mixed in a stone mortar (Khalva Yantra).  This homogeneous mixture is then subjected to Bhavana (triturating with liquid media) using freshly extracted Aparajita Swaras (juice of Clitoria ternatea) for 7 consecutive days, each day with one full Bhavana (levigation cycle) in a stone Khalva Yantra (stone mortar and pestle). During each Bhavana (levigation cycle), the mixture is mardana (triturated) continuously until the swaras (herbal juice) is completely absorbed, ensuring deep impregnation of the juice and transformation of the formulation into a soft, semi-solid mass. After completing 7 Bhavanas (levigations), the mass is rolled into 1 Ratti (approx. 125 mg) tablets manually or using a tablet-making apparatus. These tablets are then dried in the shade (chhaya sooksha) to preserve the volatile and thermolabile principles. Finally, the dried tablets are stored in airtight glass containers (Kanch-Kalash) to avoid oxidation and moisture contamination, thus maintaining the potency and shelf life of the Rasayana (rejuvinating) formulation.
