Transient Ischemic Attack Treatment in Ayurveda with Herbal Remedies

ABSTRACT

The heart and brain are the most basic units of our system. If some ask why so? Then obviously without a heart blood cannot be pumped inside a body without blood the body can not survive. Transient Ischemic Attack, Mini Stroke

INTRODUCTION

The transient ischemic attack also called a mini-stroke is the result of a temporary disruption in blood supply to a particular part of the brain. This disruption results in a lack of oxygen to the brain. As it happens, this situation can result in the appearance of many symptoms similar to a stroke. The symptoms a patient may experience include speech and visual disturbances, numbness and weakness in the face, legs and arms. But the positive part of TIA or transient ischemic attack is it does not last long as a stroke. The symptoms or effects of TIA or transient ischemic attack resolve in minutes to a few hours and fully resolve in 24 hours.

CAUSES OF TRANSIENT ISCHEMIC ATTACK

The causes of this condition are very common according to modern science. These causes are not different from the ones we generally hear in our day-to-day life. These are the following:

  • Smoking– The most common one
  • High blood pressure – Also can be due to smoking
  • Obesity– The third commonest reason
  • High levels of cholesterol
  • Regular intake of large amounts of Liver
  • Irregular heartbeat also called atrial Fibrillation
  • If the patient has diabetes– It has a very bad impact on the circulation 

As mentioned in the key points about TIA or transient ischemic attack, it is the result of aggravated Kapha dosha along with dushti of rakta. This results in blockage of Vata movement thus causing this transient ischemic attack.

SYMPTOMS OF TRANSIENT ISCHEMIC ATTACK

  • Complete paralysis of one side of the body (hemiplegia)

PATHOPHYSIOLOGY OF TRANSIENT ISCHEMIC ATTACK

Pathophysiology of transient ischemic attack depends on its subtypes. The most common issue which is observed in all types is the transient interruption of arterial blood flow to an area of the brain supplied by the particular artery. The subtypes are as follows:

  • Large artery atherothrombosis
  • Small vessel  ischemic disease
  • Cardiac embolism
  • Cryptogenic

DESCRIPTION OF TRANSIENT ISCHEMIC ATTACK TYPES

  1. Large artery atherothrombosis - This can be intracranial and extracranial. The mechanism may be a lack of blood flow distal to the site of arterial stenosis.
  2. Small vessel  ischemic disease - The underlying pathology in this case is lipohyalinosis. The commonest risk factor is hypertension followed by diabetes and age.
  3. Cardiac embolism - It is defined as the clot in the cardiac chamber generally in the left atrium and is secondary to atrial fibrillation.
  4. Cryptogenic - This is a cortical pattern of ischemia. This is without any cardiac source of emboli.

EPIDEMIOLOGY OF TRANSIENT ISCHEMIC ATTACK

  • The incidence of Transient ischemic attack in a population is very difficult to estimate due to other mimicking disorders.
  • But statistics show that about 1.1 per 1000 in the current population of the United States only.
  • The incidence rate Of Transient ischemic attack in India is 119-145/100,000 based on recent population studies.

DIAGNOSIS OF TRANSIENT ISCHEMIC ATTACK

The diagnosis of transient ischemic attack is done through following ways:

  • Physical examination and tests: Physical examination and tests are performed using a stethoscope, ophthalmoscope and others.
  • Carotid Ultrasonography
  • Computerised tomography (CT) or Computerised tomography angiography (CTA) scanning
  • Echocardiography
  • Arteriography

TREATMENT OF TRANSIENT ISCHEMIC ATTACK

The purpose of medication in TIA (transient ischemic attack) is to decrease the likelihood of a stroke. As discussed the causes of TIA one can provide treatment for the hyper coagulation going on in the blood. The blood has a tendency to clot called as coagulation which is to be reduced in this case. Rest the medication may also depend on the location, severity and type of TIA. the medicine generally given in case of transient ischemic attack are as follows:

  • Anti platelet drugs: Affect the platelet by reducing their binding capacity to each other.
  • Anti coagulants: Do not affect platelets but have impact on clotting system proteins instead of platelet function

Surgery Procedures For Transient Ischemic Attack

  • Or else surgery can be performed if the carotid artery is moderately or severely narrowed.in this case generally carotid endarterectomy.
  • Angioplasty can also be done on the carotid artery.

AYURVEDIC PERSPECTIVE OF TRANSIENT ISCHEMIC ATTACK

As mentioned before Ayurveda understands this condition as a result of vata and kapha dosha vitiation along with presence of ama dosha and rakta dushti. There is a possibility that there is presence of ama dosha only and thus it results in blockage and dushti of rakta dhatu.

The properties of ama and kapha are almost the same, that's why it becomes difficult to differentiate. Though the treatment protocol for excessive kapha and accumulated ama dosha is similar. There is also another possibility that both ama and kapha dosha are present. These bunch of possibilities can vary in different people.

But after these different possibilities there is one thing which is very common in TIA or transient ischemic attack and that is involvement of vata dosha. The ischemic attack cannot happen without involvement of vata dosha. Due to the huge vega of vata the dushit rakta circulates in the dhamniya or arteries or blood vessels and hence ischemic attack occurs due to the impurities present in blood. This blockage can happen anywhere, but when it particularly affects the vessels supplying to the brain or mastishka then it is called a transient ischemic attack.

The causes for this aggravation of dosha and dushti of rakta dhau along with accumulation of ama dosha is the wrong diet a person follows along with wrong lifestyle. A person having a good living style or lifestyle but wrong dietary habits can develop transient ischemic attack. Even in the viceversa situation something can happen. That's why it is very important that one should try to maintain a healthy diet or eating habits along with a healthy lifestyle.

Moving forward, on the Ayurvedic formulations and herbs that can help in this situation. Then mainly kapha anulomaka, vata anulomana, ama pachan and rakta shodhak treatment is done. The above verse means incompatibility of food, unclean or contaminated food, possession of spirits, gods, teachers along with mental trauma due to recurrent fear, exposure to fear and adopting difficult posture are the most general causes of Unmada occurrence.

तैरल्पसत्त्वस्य मलाः प्रदुष्टा बुद्धेर्निवासं हृदयं प्रदूष्य|
स्रोतांस्यधिष्ठाय मनोवहानि प्रमोहयन्त्याशु नरस्य चेत||

The verse suggests causative factors result in the dosha aggravation in the person who is having low level of sattva guna (Heena sattva). Low levels of sattva guna means weak minded people (In the simplest language). Then in aforementioned people vitiation of hridya (Seat of intellect) takes place. Thus the manovaha srotas (Channels carrying mental factors) delude a person's mind quickly.

Types of Somatoform Disorders

Somatoform disorders are distinguished by thoughts, emotions or actions which are related to somatic symptoms. Following are the types of somatoform disorders:

  1. Somatization Disorder: This occurs when a person continuously complains of physical symptoms irrespective of any physical condition. The diagnostic criteria includes experienced inexplicable physical symptoms that starts before the age of 30, persistence of symptoms for several years. Symptoms can include pain, stomach problems, sexual or neurological issues.
  2. Conversion Disorder: The condition is diagnosed when physical symptoms mimic symptoms of a neurological disorder when there is no neurological disorder present.
  3. Pain Disorder: Somatoform disorder characterised by pain in one or more parts of the body without any known cause.
  4. Hypochondriasis: This occurs when the person believes that his/her minor signs or symptoms are the result of a major disease.

Symptoms related to Somatoform Disorders

Modern science suggests that somatoform disorders have symptoms as following:

  • Persistent worrying about possible illness.
  • Interpretation normal bodily sensations as an indication of a severe disease.
  • Fear that the symptoms are life threatening or dire, even when there is no medical confirmation for the same.
  • Excessive visits to a physician or hospital.

Ayurveda suggests following as Somatoform Disorder Symptoms

धीविभ्रमः सत्त्वपरिप्लवश्च पर्याकुला दृष्टिरधीरता च|
अबद्धवाक्त्वं हृदयं च शून्यं सामान्यमुन्मादगदस्य लिङ्गम्||

The above verse suggests Abnormal intellect, Psychic agitation, eyes restlessness, impatience, confused speech, feeling of emptiness in mind are the most general symptoms explained in ayurveda regarding somatoform disorders.

Diagnosis of Somatoform Disorder

The somatoform disease is only diagnosed if the patient is displaying at least one somatic symptom like presence of pain or sleepiness that causes distress or negatively impacts daily routine.

The patient will have extreme and unrelenting thoughts along with severity of their symptoms, experience high levels of anxiety regarding their health and also spend too much time on symptoms or health.

Such symptoms must be present at least for six months or more, so that the patient can be diagnosed with somatoform disorder.

Treatment of Somatoform Disorder

Various therapies are recommended for somatoform disorder. The therapies include the following:

  • CBT (Cognitive Behavior Therapy)
  • Mindfulness-based interventions
  • Acceptance and commitment therapy
  • Relaxation therapy

INTRODUCTION

It is a mental disorder which is characterized by physical illness and injury and is usually known as somatic symptom disorder and somatization disorder. 1. Somatization Disorder

It varies from person to person and there is no physical condition. It persists for several years and involves pain, stomach issues and various neurological problems.

2. Conversion Disorder

It is an expression of psychology which involves loss of physical functions and has various neurological symptoms and is commonly known as Functional Neurological symptom disorder). It includes symptoms such as-

Vata doshas are responsible for the function of all systems and maintain the whole body and its subtypes are Prana, Udana, Vyana, Samana and Apana. There is a unique concept in ayurveda which is explained well as Avarana. Avarana means covering and free-flowing Vata is obstructed with Pitta and Kapha, various tissues and excreta. In Avarana, Vata gets blocked by many things. The symptoms of Pitta and Kapha are not due to vitiated pitta and kapha but are due to pull by the force of Vata. Usually, there are 2 mechanisms of vitiation of Vata-

  1. Marga Avarana
    • Obstruction in the movements of Vata by tissues.
    • Obstruction of Vyana Vayu by Prana Vayu.
    • Withholding the natural urges of the body leads to an aggravation of Vata which obstructs its pathway.
  2. Dhatu Kshaya It is the depletion of the tissues which obstructs the passage and transportation.
    • Damage to various tissues
    • Deficient nutrition for tissues

This leads to Prakopa of Vata Doshas and enormous depletion of tissues. When Prana Vayu gets obstructed there are symptoms which are manifested as below-

CHIKITSA

In ayurveda, there are three types of chikitsa-

  1. Daivavyapasraya Chikitsa - It is the chikitsa which refers to performing measures like Mantra (incantation), Ausadhi (Wearing herbs) and Mani (Wearing gems). It is helpful in All types of Manasvikara in both Psychosis and Neurosis.
  2. Yuktivyapasraya Chikitsa (Rational therapy) - It refers to the use of Ahara (diet) and Aushadi (drugs). This Antahparimarjan Chikitsa, it is done with -
    • Samshodhana Chikitsa - Tridoshas are deranged with Raja and Tamas so purification therapy is suggested which maintains the homeostasis in the body. Various treatment procedures are - Vaman (emetic therapy), Virechan (Purgation), Nasya (Nasal Therapy) and Abhyanga (massage).
    • Sanshamana (Palliative Therapy) - When purification therapy is completed Shamana and Rasayan procedures are done to arrange the mind to normal. In this Rasaushadhi, Vati, Churna and Ghrit are given.
  3. Satvavajaya Chikitsa (Psychobehavioral therapy) - This therapy is to restrain the mind and in this Dincharya (daily regimen), Ritucharya (seasonal regimen), Sadvritta (code of conduct of life) and Achar Rasayan are followed.

Hence, these tissues cause an increase in the size and weight of the uterus hence resulting in the bulky uterus. These tissues are noncancerous and usually develop during pregnancy or the childbearing years. These fibroids don't have any side effects but it causes pain during menstruation, heavy bleeding and pain in the pelvic region. Genetic disorders, early puberty are the reasons for the growth of these fibroids.
  • Post Pregnancy - Women's uterus enlarges at the time of delivering a baby through a C-section. There is a growth of muscle mass in the specific area of the uterus called Adenomyosis. Pancolitis (also called Pan-ulcerative and total colon) is a type of ulcerative Colitis in which entire colon becomes inflamed and disease spreads throughout large intestine from cecum to rectum. It is a very severe type and chronic in nature.

The inflammation destroys the cells that normally line the colon wall. As a result, sores and ulcers develop in those damaged areas. Extend of inflammation may vary in degrees up into the upper parts of the colon. That's why it is important to know how much of the bowel in the disease is affected.

WHAT ARE THE SYMPTOMS OF PANCOLITIS?

WHO GETS PANCOLITIS?

The disease usually begins in people aged 15 and 30 although it can appear at any age. It seems that both men and women are equally affected by ulcerative colitis. Approximately 20% of patients present with Pancolitis.

WHAT CAUSES PANCOLITIS?

Abnormal activation of the immune system in the intestine: Ulcerative colitis is an autoimmune disease. Normally the body's immune system protects you from infection. In autoimmune disorders, this goes away and the immune system attacks own healthy body tissues. In Pancolitis, tiny ulcers develop on the entire internal lining of the colon and these may bleed and produce pus.

Genetic factors: Ulcerative colitis does run in families. As many as one in five individuals with UC will have another family member with UC. Ten percentages of identical twins both develop UC. If you have Pancolitis, there is an increased chance that your children, brothers and sisters will also develop the disease.

Environmental factors: It includes intestinal infections, air pollution, certain diets, sanitation and hygiene, where you live in the world, and use of certain medications.

Certain foods and beverages can make the symptoms of Ulcerative colitis worse. These may include the following:

  • Dairy products
  • High fiber foods (like beans, bran, nuts and seeds.) and Vitamin B6
  • Alcohol
  • Fried items and fatty foods.
  • Caffeine
  • Carbonated beverages
  • Stress can make the symptoms of UC worse.

CAN PANCOLITIS CAUSE COMPLICATIONS?

Some patients do have complications in their skin, eyes, joints or liver as a result of their UC disease. People with pancolitis have a slightly higher risk of developing colon cancer than the general population. As a result, cancer screening with colonoscopy is recommended in all patients after the disease has been present for eight to 10 years.

C. Brahmi Ghrit – 1 teaspoonful at night with milk or in vegetable soup.

There is an interesting case I would like to mention here. The lady is 48 years old , suffering from Pheochromocytoma ( cancerous) and was having big tumor inside her urinary bladder. The diet in Pheochromocytoma and other cancers should be strictly alkaline diet. Do not consume citrus fruits – lemons, oranges. Every evening vegetables soup should be consumed. There is difference of opinion about alkaline and acidic diet. All natural fruits, ( except citrus), should be considered alkaline. All acidic fruits – lemons and oranges should be avoided. The acidic food like non-veg diet, proteins, yogurt, ( all sticky food), fire element disturbing diet, alcohol should be avoided at all cost.
