Ayurvedic Treatment for Ulcerative Colitis with Herbal Remedies

ABSTRACT

Ulcerative colitis is described as a type of inflammatory bowel disease(IBD), the other type is Crohn’s disease. In ulcerative colitis, there is inflammation and ulceration of the colon or rectum. The symptoms will range from light to severe. The etiology is unknown but can be due to an autoimmune condition, or genetic disorders. In some diseases like polyarthritis, sacroiliitis, spondylitis, inflammatory liver diseases, and various skin lesions ulcerative colitis was also commonly seen. Let’s discuss it in detail!

INTRODUCTION

Ulcerative colitis is an autoimmune condition, which can be an acute and chronic disorder of unknown etiology characterized by inflammatory mucosal ulcerations of the colon and rectum. Crohn's disease and ulcerative colitis are types of inflammatory bowel diseases (IBD). Ulcerative colitis mostly begins in the rectosigmoid colon but often extends to affect the entire colon. In Crohn's disease, the inflammatory changes are granulomatous/ granulomatous colitis (a chronic inflammatory condition that affects the colon or large intestine) in Ulcerative colitis the inflammatory changes in the colon are nonspecific. The causative mechanisms of both diseases are common.

CAUSES

The specific cause of ulcerative colitis is unknown but some causes are as follows -

  • Abnormal Immune Response - It's an autoimmune condition where the T-cells penetrate the colon and mistakenly attack the healthy cells lining the colon causing inflammation and ulceration in the colon.
  • Genetic Factor - Some research shows that certain genes increase the chance of developing ulcerative colitis. Some regions of the genome linked to cause UC as 12, 16, and 19 but the disorder is influenced by multiple genes.
  • Environmental Factors - Here the factors include, diet, breastfeeding, medication, stress tobacco use, and use of more dietary substances can lead to inflammation of the colon. More intake of vitamin B6 and unsaturated fat may enhance the risk of ulcerative colitis, and also alcohol use eating meat.
  • Microbiome - The good bacteria, fungi or viruses in our gut that help in digestion is called the microbiome, are lacking or in a weak stage if compared with a healthy man and a person suffering from IBD (Inflammatory bowel disease).

SYMPTOMS

  • Abdominal pain
  • Blood and mucus in diarrhea can be seen for days, weeks, or months.
  • In most cases, bloody diarrhea contains stringy mucus, lower abdominal pain, and cramps, usually relieved by defecation.
  • Sometimes fever and weight loss (an asymptomatic interval can be seen for some time after which stress physical or mental may typically cause a flare-up)
  • Toxic Megacolon - sudden cessation of bowel function with the production of toxic dilatation of the colon
  • The bowel may undergo fibrosis with narrowing of the lumen.
  • Sometimes deranged electrolytes due to blood in stools, and rectal bleeding.
  • Dehydration
  • Loss of appetite
  • Rectal tenesmus - Incomplete defecation
  • Pancolitis - enlargement of the entire large intestine

DIAGNOSIS

Diagnosis can be done by doing:

  • Laboratory findings
  • Imaging
  • Physical examination

1. Laboratory Findings

  • FOBT (Fecal occult blood test) detects a small amount of blood in stool, Leukocytosis, Anemia (due to chronic blood loss), Thrombocytosis
  • Serum ferritin, ion and total iron-binding capacity, and transferrin saturation
  • Serum albumin may be low due to inflammation
  • The fecal calprotectin test will be elevated in inflammatory conditions affecting the colon
  • Complete blood count (CBC)- look for Anaemia (decrease in hemoglobin and RBCs) and thrombocytosis (increase in platelet count)
  • Serum electrolytes for severe dehydration, hypokalemia, and KFT(kidney function test) for kidney injury
  • LFT (Liver function diseases) form involvement of bile duct
  • A stool culture to check for infectious colitis
  • ESR(Erythrocyte sedimentation rate) or c-reactive protein to see for inflammation.

2. Imaging

  • X-ray and CT scan to detect perforation or toxic megacolon
  • Bowel Ultrasound
  • MRI is useful to detect PSC (Primary sclerosing cholangitis)
  • Sigmoidoscopy or colonoscopy to view the lining of the rectum, and sigmoid colon for any inflammation.

3. Physical Examination

  • Physical examination includes vital monitoring
  • Bowel sounds and seeing for tenderness in the abdomen.
  • Check for bloating and pain while palpating
  • Digital rectal exam to check for blood or mucus in stool.

STAGES OF ULCERATIVE COLITIS

The stages of colitis are based on the severity, which can be mild, moderate, and severe:

  • Mild - Person has less than or equal to 4 stools per day with minimal systemic symptoms
  • Moderate - More than 4 times stool per day and moderate symptoms
  • Severe - More than 6 stools with blood per day systemic symptoms like fever, low hemoglobin, increased heart rate, and life-threatening condition

The extent of inflammation in the colon can also be classified using the Montreal classification system with stages:

  • Proctitis (E1) - Inflammation limited to the rectum.
  • Left-sided colitis (E2) - Inflammation reaching the splenic flexure.
  • Pancolitis (E3) - Inflammation affecting the entire colon.

MANAGEMENT

  • Corticosteroids - Reduce inflammation and suppressing the immune system
  • Aminosalicylic acid like, 5 Amino Salicylic acid - Treat Inflammation in the colon and small intestine
  • Biologics - Used for modifying the immune system.
  • Iron Supplementation - Used to maintain hemoglobin, iron level in the body happened due to blood loss
  • Anticholinergics - Sometimes used to treat abdominal cramps
  • Immunosuppressant therapy and Vaccination - Many patients need immunosuppressant therapy as it is an autoimmune disorder so patients may also require different vaccinations for influenza, hepatitis B, chicken pox herpes zoster diseases, etc.

AYURVEDIC ASPECT

Etiology and pathophysiology

In Ayurveda Ulcerative colitis can be related to Raktaatissar (bloody diarrhea with stools). There are some etiological factors explained in Ayurvedic texts regarding Raktajatisar like food that is heavy for digestion (guru), food that has apposite properties (cold potency food and hot potency food intake together), Koshtha (gut), desha (Region or surrounding areas), Intake of meat of Pig, buffalo, fish mixed with oil cake, Intake of pastries in excess after food, unboil milk intake in excess, Intake of excess Katu rasa (Pungent thing), Amla (Sour taste), Lavana (Sour taste), hot potency food intake in excess, Tikshana (Sharp acting things). Because of these etiological factors and the presence of Ama dosha (toxic substance due to indigestion) the Pitta dosha aggravates in Kostha which agitates the inner lining of the large intestine by which the mucosal stem cells get harmed, and there is blood in the stools.

Pathway of diseases and prognosis

The manifestation of this disease is in the lower tract where the body is dominated by Vata (Urethra and anus due to contact with vitiated Vata) and vitiated Pitta which does the Dushti of Rakta which damage to the inner lining of the large intestine and leads to blood in stools.

Treatment According to classic texts

  • Goat’s milk boiled with Padma (Nelumbo nucifera), Utpala (Nymphaea alba), Samanga (Mimosa pudica), Mocarasa (Bombax ceiba) or with Sariva (Hemidesmus Indicus), Yashti (Glycyrrhiza glabra), Lodhra (Symplocos Racemosa Roxb.) or with sprouts of vata (Ficus Benghalensis), etc mixed with honey and sugar is beneficial for use for drinking, as food and for bathing the rectum
  • Meat soup which is not sour and mixed with ghee used for drinking and along with food is beneficial.
  • Paste of Shatavari (Asparagus Racemosus) mixed with milk should be consumed by the person who takes milk only as food, this cures bleeding diarrhea soon, also the medicated ghee of Shatavari (Asparagus Racemosus).

Why did my doctor recommend colonoscopy? What symptoms indicate a need for a colonoscopy?

Colonoscopy is broadly used for the diagnosis and treatment of colonic disorders (Range of diseases, including inflammatory bowel disease – Crohn's disease and ulcerative colitis – diverticular disease and colon cancer. Colonoscopy, in which the doctor looks inside the colon of the patient with a thin, flexible tube that holds a tiny camera and light. Colonoscopy is one method that doctors use to diagnose UC and Crohn's diseases. A colonoscope is a long, thin flexible tube with a camera at the end. The doctor uses it to view the inside of your colon and rectum. In this article, we are going to put a lime light on when a patient is in need of a colonoscopy.

Colonoscopy

INDICATIONS FOR COLONOSCOPY

  • In lower Gastrointestinal bleeding
  • For the screening of colorectal polyps
  • Help to diagnose colon cancer, colorectal cancer
  • Acute and chronic diarrhea
  • Abnormal radiological examination
  • In unexplained abdominal pain
  • Long term chronic constipation

Inflammatory Bowel Disease (IBD) includes two conditions (Crohn's disease and Ulcerative colitis) that are characterized by chronic inflammation of the gastrointestinal (GI) tract. Long term inflammation results in damage to the GI tract. So we need to clear out the exact difference between Crohn's disease and ulcerative colitis and the need to do colonoscopy.

Ulcerative Colitis

Ulcerative colitis is an inflammatory bowel disease (IBD) that causes inflammation and sores  in the digestive tract. Ulcerative colitis affects the innermost line of intestine (colon) and rectum. Occurs in the large intestine (colon) and the rectum.

Symptoms:-

The most common symptoms are diarrhea, bleeding from the anal route and pain in the abdomen.

A colonoscopic finding in ulcerative colitis is swelling, redness, and sores in the intestine. It can show the severity of disease. It will help the doctor to make a correct diagnosis with a colonoscop

Endoscopic Score Of Ulcerative Colitis:-

  • Score 0 - Normal
  • Score 1 - Mild disease
  • Score 2 - Moderate disease
  • Score 3 - Severe Ulceration

Crohn's Disease

Crohn's disease is a disorder mediated by t -lymphocytes cells which arises in genetically susceptible individuals because of  breakdown in the regulatory constraints  to enteric bacteria. Can affect any part of the GI tract (from the mouth to the anus) - Most often it affects the portion of the small intestine before the large intestine/colon.

Symptoms:-

  • Abdominal pain is the most common symptom
  • Chronic diarrhea
  • A feeling of fullness
  • Fever
  • A loss of appetite
  • Weight loss is a common symptom
  • Abnormal skin tags (usually on patient’s buttocks)
  • Anal fissures can be there

BASIC SYMPTOMS OF IBD WHEN A DOCTOR THINKS ABOUT COLONOSCOPY

Many of the same symptoms can indicate other problems, but in long term if following common symptoms persist then doctor may advise colonoscopy in Ulcerative Colitis Or Crohn’s Disease.

  • Abdominal Pain
  • Bloating
  • Chronic Fatigue
  • Blood in Stools
  • Narrow Thin Stools
  • Persistent Diarrhea
  • Constipation
  • Change in Bowel Habits
  • Weight Loss
  • Cramps

COLONOSCOPIC FINDINGS OF IBD

Cobblestoning (Uncommon finding) mucosa and linear ulcers characterize the endoscopic appearance of Crohn's disease. Inflammatory bowel disease (IBD) can be broadly classified into Crohn disease (CD) and ulcerative colitis (UC). It is a chronic idiopathic (Cause unknown) inflammatory disorder badly affecting the gastro-intestinal tract. Terminal ileum is not predominantly involved in Ulcerative colitis but is commonly involved in Crohn's diseases.

AYURVEDIC PERSPECTIVE OF IBD

Rakta Atisara

The vitiated pitta dosha and continuous hetu sevana cause depraved blood and hence rakta atisara. The symptoms include Daha i.e burning sensation, pain, thirst and guda pak i.e rectal abscess, rectal bleeding (Ch. Chikit. 19). Rectal blood is mixed with fecal discharge. The rise in symptoms is due to regular and excessive intake of pitta vardhak aahar (food that vitiates the pitta dosha).

Grahani Rog

The vitiated agni (agni mandya) in grahani is grahani rog.

The food we take in the form of Aahar which is important for the nourishment of DHATU (7 dhatus), AGNI is the main cause of that nourishment. Because if agni gets vitiated, the digestion of food and hence proper nourishment will not be there.

Types Of Grahani Rog:-

  • Vataj Grahani (Ch. Chikitsa 14/60-62) - The patient suffering from Vataj grahani has depraved digestion and sour outcome of rasa pak after meal, Digestion impaied, Feeling dry,  Excessive thirst and hunger.undigested food passed in the form of vomitus and in stool, difficulty in defecation and stool passed is usually dry or watery sometimes, Aamyukt (presence of undigested food), coughing and breathing difficulties are other prominent symptoms of vataj grahani.
  • Pittaj Grahani (Ch. Chikitsa 14/65-66) - The skin color of the patient gets pale. The stool passed is usually yellow and sometimes blue. Rotten food like burp,  Pain in chest,  Anorexia ,  Thirst.
  • Kaphaj Grahani (Ch. Chikitsa 14/67-70) - Troublesome digestion   Anorexia ,feeling of vomit,   Sweetness in mouth, Coughing Stool passed is usually undigested ,kapha mixed and heavy as compared to vataj and pittaz grahani.
  • Tridosha Grahani (Ch. Chikitsa 14/72) - Tridosha grahani means assimilation of all three types of doshas and hence assimilation of symptoms of all the doshas.

Classical Medicines / Home Remedies / Combinations:-

  • Use of Goat Milk - Cold milk of goat mixed with honey and sugar can be used to wash the anal region after defecation (Charak 15)
  • Fruit Soup - The patient of ibd can be given the soup of Gambhari fruit (Gmelina Arborea) with juice of pomegranate. (Charak chik)
  • Powder of Neelkamal, Mochras, Lajjavathi, Kamal kesar  in equal quantities can be used in the patient of ibd. (Cha Chik. 15)

Grahani Roga & its relation with Intestinal Diseases in Modern Language?

Do you often get disappointed because of unexpected gut symptoms? Do you often experience pains and cramps in the abdomen? Do you frequently get exposed to changes in bowel habits? Do you often feel a sudden and urgent need to use the bathroom? Well, you may be confronted with Grahani roga which can be pretty irritating. From having to backtrack your diet to the awkwardness of explaining your situation to others, no one understands your situation except for the ones who have already acquainted with them.

Most people today follow a sedentary lifestyle that leaves them at the stake of irregular diet and diet timings. Added to that is the tremendous mental stress that people undergo in these modern days. All these eventually lead to a diseased state of body and mind. Disorders of the digestive system play a major role among them.

Grahani Roga, Intestinal Diseases, IBS, Ulcerative colitis, Irritable Bowel Syndrome

GRAHANI ROGA - THE AYURVEDIC CONCEPT

Agni, the body fire is solely responsible for our lifespan, health, immunity, energy, enthusiasm, vital breath, luster, corpulence, and heat processes. When the body fire functions properly, one lives long and free from disorders. Diseases occur when this fire is deranged and death occurs when this fire is extinguished. Hence the root cause of all is Agni. - Charaka Samhita

Within the process of digestion lies the root cause of all the diseases. When the digestive system is working at its optimum supported by the correct lifestyle and correct quantity and quality of food as per our individual constitution, the food we ingest is broken down into its constituents, nourishes our tissues, and supports our everyday functions.

Agni is a unique concept of Ayurveda and is solely responsible for the physiology of digestion. Following an incorrect diet and food habits can derange our digestive fire which ultimately affects the digestive process, thereby leading to the formation of toxins.

Grahani may be related to the seat of this digestive fire. According to Ayurveda, Grahani is accountable for the ingestion, digestion, absorption, and assimilation of food. Grahani is among the major causes of digestive illness globally, especially in developing countries, and is often constituted because of improper diet and lifestyle habits.

To put it another way, Grahani is caused by the incompatibility of factors that are related to digestion and metabolism. Ayurveda insists that Grahani is the site of Jatharagni (digestive fire). Accordingly, any factor that may cause derangement of this Jatharagni can potentially lead to Grahani Roga.

TYPES OF GRAHANI ROGA AND THEIR SYMPTOMS

  1. Vataja Grahani Roga results from increased Vata. It may be due to a variety of causes that include suppression of natural urges, intake of fermented foods, intake of contaminated, pungent, bitter, astringent, and dry foods. Symptoms being the absence of hunger, noise in the ears, pain in the flanks, thighs, and neck, vomiting, depression.
  2. Pittaja Grahani Roga results from increased Pitta. It may be due to the intake of too many raw foods that are pungent and sour in taste. The patient may eliminate foods that are not properly digested, stools may be deep yellow in color, and may experience burning sensations in the chest or throat.
  3. Kaphaja Grahani Roga results from increased Kapha. It may be due to the increased use of fatty, heavy, cold foods. Also sleeping right after meals results in improper digestive function. Patients may experience excess coating in the mouth and tongue, sweet taste in the mouth, heaviness in the abdomen, elimination of mucus along with feces.

HOW IS GRAHANI ROGA RELATED TO IRRITABLE BOWEL SYNDROME AND OTHER INTESTINAL DISEASES?

When the process of digestion is affected, diseases arise in the gastrointestinal tract. According to Ayurveda, the function of the duodenum is twofold. It takes part in the digestive process and it also holds on to the food until it is digested. Once the digestion is completed, the duodenum releases the food into the colon from where it gets excreted under the influence of Apana Vayu.

Grahani is caused by the derangement of digestive fire and causes weakness in the duodenum. As the digestive fire remains deranged, the functions of the duodenum are inhibited. The duodenum will either release its contents earlier than normal into the colon or it will retain its contents for longer periods. This eventually results in the food released into the colon either being uncooked or overcooked.

In response to this condition, one may experience loose bowel movements alternating with constipation. Since these are the cardinal symptoms of Irritable Bowel Syndrome, Grahani Roga can be related to Irritable Bowel Syndrome, Ulcerative Colitis, and other Intestinal Diseases, the symptoms of Grahani roga being more or less similar to the symptoms of these diseases.

MANAGEMENT OF GRAHANI

Following the general principle of treatment, the offending dosha must be brought into equilibrium foremost. The management of Grahani should ultimately aim at correcting Agni and strengthening the duodenum. Consumption of buttermilk is considered to be effective towards treating all types of grahani as buttermilk is easy to digest. It is also known to balance the three doshas and strengthen the digestive fire.

Digestive and carminative agents can be given to stimulate the digestive fire. Once the digestive fire is strengthened, the next concern must be to get rid of the toxins. This can be made possible through reducing therapies and elimination therapies that include emesis, purgation, enema, and other detox therapies.

The preventive measures include avoidance of causative factors of Grahani Roga. Avoid intake of katu (pungent), tikta (bitter), kashaya (astringent), and rasa pradhana ahara (taste dominant food). Excessive guru (heavy), sheeta (cold), snigdha (oily) foods must also be avoided. Sleeping during the day and suppression of natural urges are also to be avoided.

In a nutshell, by following a healthy and balanced diet and lifestyle, all digestive disorders including grahani can be avoided. By every effort, the digestive fire of a person must be protected because when the fire is lost, death occurs. When the fire is deranged by doshas, diseases occur. When the fire is well maintained, the person leads a healthy and happy life free from diseases. Agni is responsible for the health of an individual. - Ashtanga Hridaya

MORE ON ULCERATIVE COLITIS & AYURVEDA

Ulcerative colitis is considered as an inflammatory disease that affects intestines by creating inflammations which produce small sores called ulcers within the lining of the intestines or gastrointestinal tract. This condition is said to begin at the rectum and gradually spread to intestinal areas. The main reason behind the pain is attributed to the ulcers formed and the fact that due to damages caused to cells lining the tract surfaces, mucous production could be reduced. Bleeding is also caused due to the same reasons of ulcer formations and absence of mucous production.

In Ayurveda, Ulcerative colitis is actually related to Pitta dosha, one among the three of the Tridosas i.e., Vata, Pitta and Kapha. Related to the same, it is called Pittaja grahani in some concepts. Here the second term Grahani itself indicates that it is something in association with intestines and it is indicated that it is caused due to Pitta as its mentioned as Pittaja grahani. This is caused due to the vitiation of Pitta in the intestinal tracts and Vata sometimes. The idea of Purisha Vaha srotas is also to be mentioned here as Purisha is the feces that faces difficulty to be passed down in cases of constipation as an effect. The Srotas here is evidently the tract which carries the feces which is the intestinal tract.

CAUSES INCLUDE

Though it could be possible to address many related causes, still the exact reason remains unsolved to be stated properly. But this stays satisfactory as these reasons are always found enough to heal the disease when once diagnosed with. Few of them include the primary causes like diet. So as to acknowledge the reasons of a disease as anything related to the diet it is always important in Ayurveda to understand terms like Doshas and Agni. Tridosha is a well-known concept of the 3 humors- Vata, Pitta and Kapha. Of these Pitta being the one related to ulcerative colitis, it is important to understand that the primary step to be taken is to prevent the vitiation of the Dosha by forming a daily diet accordingly, or else foods that imbalance the Pitta dosha results in the Ama condition. Ama is the deposition of toxins on intestinal tracts due to indigestion of food, as the vitiation of Pitta has hindered proper action of Agni.

Other than this aspect, it is also considered as a genetically inherent disease and has a chance to pass down to you through the parental genes.

HOW TO IDENTIFY?

The symptoms that might help you confirm your disease includes abdominal pain, sense of inappropriate bowels, loss of interest to eat followed by weight loss and low energy, rectal pain, rectal bleeding, diarrhea with blood or pus content and fever. Living with the disease is hard and so once you recognize the symptoms, it's important to seek advice from your physician to treat it.

TYPES OF ULCERATIVE COLITIS

  1. Ulcerative Proctitis - In this type, the inflammations are only established at the rectal area and bleeding is the prime symptom.
  2. Pancolitis - This is the type in which the entire colon gets affected and leads to some serious symptoms like heavy diarrhoea with bleeding, abdominal cramps and loss of weight.

MAJOR COMPLICATIONS

Intestinal perforation, acute bleeding, osteoporosis, increased risk of colon cancer, lagged growth in children, high levels of dehydration etc.

Toxic Megacolon: A Complication Of IBD

The large intestine is the lowest part of the digestive tract which consists of appendix, colon, and rectum where re-absorption of water occurs from the food and processing of waste products along with its elimination in the form of stool through the anus.

There can be various reasons for the malfunctioning of the large intestine. One of them is toxic dilatation of the colon or toxic mega colon. Abnormal dilatation of the mega colon is the anomaly for which these terms are used. It is a very serious medical condition which needs to be corrected as soon as possible otherwise results in severe signs and symptoms. Although it is a rare condition in which within a small span of time, widening of the large colon or intestines occur and can prove to be fatal. It is usually presented as a complication of the IBD, i.e. Crohn's disease or Ulcerative colitis.

toxic mega colon, toxic megacolon

WHAT CAN BE THE CAUSES OF TOXIC MEGACOLON?

Inflammatory bowel disease is the main cause of toxic mega colon. IBD comprises of ulcerative colitis and Crohn's disease. In both of them, there is irritation and swelling of the intestines, both large and small. Both of them are very painful and distressing conditions which may cause severe and permanent damage to the colon. Intestines become dilated, expanded and distended in IBD that results in the inability of those to remove stool and gas present in them. The gradually huge amount of gas and waste products build up in the colon ultimately leading to rupture of the colon. This rupturing of intestines is a life-threatening condition as due to the rupturing of the colon the normal bacteria present in them may spread to the whole body causing infection and death.

In some cases, it can also occur due to infection from Clostridium difficile.

WHAT CAN BE THE SIGNS AND SYMPTOMS OF TOXIC MEGACOLON?

As it can occur within a short span of time, symptoms also appear suddenly, which include:

  • Pain in the abdomen
  • Distention of the abdomen
  • Bloating
  • Fever
  • Tachycardia i.e. increased heart rate
  • Shock
  • Diarrhea which may contain blood
  • Painful bowel movements

HOW CAN TOXIC MEGACOLON BE DIAGNOSED?

It can be diagnosed through physical examination and some lab tests. Patient case history can reveal any of the inflammatory bowel diseases. During the physical examination, abdominal tenderness can be detected. Some of the lab tests can confirm the diagnosis:

  • Abdominal X-ray
  • Abdominal CT scan
  • Blood tests like complete blood count and blood electrolytes

HERBAL REMEDIES FOR TOXIC MEGACOLON

ancient reference, shloka, digestive fire

Meaning- Digestive fire is the main of all the fires in the body. All other fires or Agni depend upon the Jatharagni as it illuminates all others. They all get strength from Digestive fire and their strength depend upon it. Hence, food consumption should be according to the nature of this digestive fire. The strong state of digestive fire provides strength, longevity, health, excitement, and intelligence to the body.

Pancolitis (also called Pan-ulcerative and total colon) is a type of ulcerative Colitis in which entire colon becomes inflamed and disease spreads throughout large intestine from cecum to rectum. It is a very severe type and chronic in nature.

The inflammation destroys the cells that normally line the colon wall. As a result, sores and ulcers develop in those damaged areas. Extend of inflammation may vary in degrees up into the upper parts of the colon. That's why it is important to know how much of the bowel in the disease is affected.

WHAT ARE THE SYMPTOMS OF PANCOLITIS?

  • Blood Discharge in stool
  • Abdominal severe pain and cramps
  • Tenesmus - It is a sensation to pass stools, even though your bowels are already empty.
  • Sudden weight loss
  • Fatigue
  • Night sweats
  • Fever

WHO GETS PANCOLITIS?

The disease usually begins in people aged 15 and 30 although it can appear at any age. It seems that both men and women are equally affected by ulcerative colitis. Approximately 20% of patients present with Pancolitis.

types of ulcerative colitis, ulcerative colitis

WHAT CAUSES PANCOLITIS?

Abnormal activation of the immune system in the intestine: Ulcerative colitis is an autoimmune disease. Normally the body's immune system protects you from infection. In autoimmune disorders, this goes away and the immune system attacks own healthy body tissues. In Pancolitis, tiny ulcers develop on the entire internal lining of the colon and these may bleed and produce pus.

Genetic factors: Ulcerative colitis does run in families. As many as one in five individuals with UC will have another family member with UC. Ten percentages of identical twins both develop UC. If you have Pancolitis, there is an increased chance that your children, brothers and sisters will also develop the disease.

Environmental factors: It includes intestinal infections, air pollution, certain diets, sanitation and hygiene, where you live in the world, and use of certain medications.

Certain foods and beverages can make the symptoms of Ulcerative colitis worse. These may include the following:

  • Dairy products
  • High fiber foods (like beans, bran, nuts and seeds.) and Vitamin B6
  • Alcohol
  • Fried items and fatty foods.
  • Caffeine
  • Carbonated beverages
  • Stress can make the symptoms of UC worse.

CAN PANCOLITIS CAUSE COMPLICATIONS?

Some patients do have complications in their skin, eyes, joints or liver as a result of their UC disease. People with pancolitis have a slightly higher risk of developing colon cancer than the general population. As a result, cancer screening with colonoscopy is recommended in all patients after the disease has been present for eight to 10 years.

WHAT TREATMENTS ARE THERE FOR PANCOLITIS?

Pancolitis can be treated by medicines but sometimes surgery may be necessary. Some people also change their diet and lifestyle to support their medical treatment such as exercise, improving quality of sleep, reducing stress. There is not certain treatment in allopathy but Ayurveda has an excellent management for the same.

AYURVEDIC CONCEPT OF PANCOLITIS

From ayurvedic view point, pancolitis can be known as pittaj Grahani. Three fundamental energies (doshas: vata, pitta & kappa) should be in balance stage to get perfect health. Pitta type people are genuinely more –prone to have this. Here, Pitta dosha gets imbalanced, obstruct intestines and causes Pittaja Grahani. Excessive consumption of Pitta aggravating Foods (such as excess of spicy, pungent, acidic, sour, alkaline nature foods, food items which produces burning sensation in the body) damage both rakta dhatu (blood tissue) and mamsa dhatu (muscle tissue). This leads to formation of ama that gets deposited between the villi in the intestines, forming a smooth coating that impairs the normal function and immunity of the intestines. Later it becomes more extensive over time and effects the entire colon called Pancolitis.

AYURVEDIC TREATMENT FOR PANCOLITIS

Ayurvedic treatment is mostly recommended by doctors all over the world. Ayurvedic treatments were both safe and effective, providing symptomatic relief of Pancolitis with no harmful side effects. Natural Cure for Ulcerative Colitis

ulcerative colitis, natural, cure, ayurveda, herbal

There is a serious disorder called ulcerative colitis and Inflammatory bowel disease ( Crohn's disease ) which is having serious physical, mental and psychological impact on one's health and personality. People are looking for answers - Is there any natural cure for ulcerative colitis?

I have seen many patients start suffering from depression, stop working, lose their jobs, can't travel and lead normal lives if they suffer from ulcerative colitis.

To find out a real solution to the problem, I looked out for solutions in ancient ayurvedic texts like Charak Samhita (2500 BC - ancient Ayurvedic text), Sushruta Samhita ( Father of Surgery- 2000 BC text ) , Bhavprakash Nighantu ( 1700 AD - herbs text ) , Sharanghar Samhita ( 1300 AD book ) , Chakradutt Samhita ( 1100 AD Book ) and many others.

The problems of loose stools, bleeding with stools, drop by drop or occult bleeding, dark stools, frothy stools with mucus and painful cramps in abdomen, wind, dyspepsia, are the symptoms described in Ayurveda for this problem of inflammatory condition of the bowel.

WHAT ACTUALLY HAPPENS TO COLON - MODERN VIEW ?

According to modern medicine, the cause of ulcerative colitis is unknown. If the cause is unknown then how can we treat it? So the disease is listed as incurable disease in modern allopathic text books.

Many books quote it's an auto-immune disease i.e. when our own Immunity starts to attack our own digestive tract. Our own army becomes our enemy. So what do you do in this situation? Kill our own army with Immunosuppressants like Azathioprine etc. or stop the fighting ( inflammation ) between Antibodies ( our immunity ) and antigens ( unknown enemy) with Steroids ?

The other drugs like Mesalazine ( 5 Amino Salicylic acid) with brand names Asacol, Mesacol, Pentasa, Lialda, Apriso are given for life long. This is also an acidic drug and can cause serious side effects like kidney disease, nausea, diarrhea, pain abdomen, bone marrow depression ( leukopenia, neutropenia, agranulocytosis, aplastic anemia, low platelets ), hair loss, rashes, drug induced fever, interstitial nephritis, nephrotic syndrome ( This list is taken from Wikipedia ) –

Steroids have their own side effects and they may cause diabetes, disturbance of hormonal axis, bone loss, muscle loss, redistribution of fats ( round face ), severe weakness and many more. Mesalazine enemas are also given sometimes.

These are temporary measures to stop the bleeding for a while but as soon as you stop the drugs like Mesalazine (Asacol, Pentasa or Mesacol) the bleeding starts again.

AYURVEDIC VIEW OF ULCERATIVE COLITIS, CROHN’S DISEASE AND INFLAMMATORY BOWEL DISEASES

In Ayurveda, these diseases are described as Grahani, Sangrahani ( many types ) Raktatisaar, Pitta Atisaar.

According to Ancient Ayurvedic text Charak Samhita these two verses explain Grahani dosha in general and in particular.

Verse - 1:

ulcerative colitis, verse, shloka

Grahani dosha means defect of agni situated in grahani.

Verse - 2:

ulcerative colitis, verse, shloka

Grahani dosajanan denotes particularly the four disorders (vataj, pittaj, kaphaj and tridoshaj) of grahani and generally mildness of digestive fire, indigestion etc.

These are all curable with difficulty as per Ayurvedic texts. When a text says it's curable, that means it's curable. So all depends upon the doctor, the combination of medicines he uses, his management skills, patient compliance, how strictly the patient follows the doctor's advice and make some dietary changes. When the book says it's curable, that means it's curable.

Anyway, in my clinical practice, I have seen many patients getting completely cured rather than going into remission. My patients are free from allopathic drugs for ulcerative colitis, free from Ayurvedic herbs given to them for few months.

What I mean to say is that the patients are getting cured ! !

I urge all the allopathic doctors, the governments to take necessary steps to popularize Ayurvedic herbs to help as many patients as possible. At present, I don't have big funding for free clinical trials to establish the truth but I am seeing it every day happening in my clinic and I am happy about it.

WHICH HERBS I AM USING TO CURE ULCERATIVE COLITIS OR OTHER IBD'S

In Ayurveda, there are special ways to use pearls and corals to heal ulcers. Ancient methods are used to make medicines from precious cooling gems like pearls, corals, agate, Amber, Salmalia malabarica resin, Commiphora mukul Resin ( Ayurvedic names - Mukta pishti, Praval Pishti, Jahar Mohra Pishti, Kaharawa or Trinkant Mani Pishti, Moch Ras, Bol Parpati ).

Other than these costly gems, (Actually not so costly) because we don't need beautiful gems to make jewelry but we need the left over to crush them and make the medicines. We just need the left over from the jewelry. I developed a formula called Pitta Balance. Ancient Vaidya's use the mixture of all these to make into small sachets but I used the vegetarian capsules to make it easy for patient compliance. It's cheap Approx. 28 $ a month!

There are herbs which are cooling and Astringent in nature. Their cooling properties balances the heat, reduced the fermentation, acid formation in the intestines and the astringent property helps to heal the ulcers. I have seen patients with huge ulcers getting healed using these herbs. I have named these formulas as -- COOLSTRIN - A and COOLSTRIN - B. I am using them in advanced stages as most of the patients respond well to the basic herbs mentioned in ulcerative colitis care pack 1 and the herbs are for initial stages are Arjuna, Kutaz and above mentioned Gems ( Pitta balance ) + Vatsakadi Churna - a Blend of herbs containing Bark of Kutaz and 6 more herbs.

The herbs like Arjuna ( Terminalia arjuna ) and Vasaka ( Adhatoda vasica ) are known to have wound healing, cooling, astringent / bitter properties. I am using Arjuna in basic pack and Vasaka in advanced stages. They help to heal the ulcers, stop bleeding. Terminalia arjuna is full of natural enzyme known for energy called - Co Q 10. So it's good for heart as well as improving overall energy levels. Vasaka was originally used for bleeding while coughing but it's good for internal bleeding as well. It has remarkable ulcer healing properties. I am using standardized extracts of Arjuna and Vasaka and other herbs in vegetarian capsules with no extra fillers or preservative or chemicals.

There is a wonderful herb and the names says it all. Halorrhena anti-dysentrica. It's known to stop diarrhea and dysentery of any type, which can be infective diarrhea or E.coli, or due to IBD or Colitis. I developed Planet Ayurveda Kutazghan Vati (standardized aqueous extract of Halorrhena antidysentrica bark). This is used in dosage of 2 tablets twice daily or 2 tablets thrice daily to immediately stop the diarrhea. The dosage can be increased further as it's just bark of the herb and nothing else. No chemcials or preservatives but only the herb. It can be used along with any other ongoing drugs like Mesacol/ Pentasa or Azathioprine or Steroids like Omnacortil or Wysolone. If you feel constipated after using Kutazghan Vati or Motion Stop tablets ( another product containing the herb Kutaz ) then you can reduce the dosage accordingly.

There is a herbal mixture called Vatsakadi Churna. I found this mixture combination mentioned in ancient Ayurvedic text as a blend of herbs to help in all forms of IBS, IBD's, Crohn's disease and Ulcerative colitis. It contains the herb powder of Kutaz bark and about 8 more herbs.

So The Ulcerative Colitis Care Pack 1 Contains:

  1. Cap. Arjuna – 2 twice daily
  2. Cap. Pitta Balance – 1 twice daily
  3. Tab. Kutazghan Vati -2 twice daily
  4. Vatsakadi Churna – 1 teaspoonful twice daily

About 70 % of the patients get relieved of the symptoms within first 10 days of using this combination. The diet also plays an important role in getting rid of the disease.

The Advanced Stages Ulcerative Colitis Care Pack:

To fortify the formula of basic pack, I needed more combination of herbs. So I developed Coolstrin-A and Coolstrin-B along with Vasaka. Instead of Kutazghan Vati, I am using Motion stop which contains Kutaz and also another useful herb called Aegle marmelos ( Bael seed).

ROLE OF DIET IN ULCERATIVE COLITIS

Diet is very important method of healing the ulcers and getting rid of toxins, stop the ongoing fermentation in the intestines. Ayurveda says this is AMA or endotoxins produced in intestines which can be understood as Ama can behave as antigens and the body reacts to them and tries to kill or neutralize them and in the process damages the intestines. Ama is a sticky material. It sticks to intestines, blocks the channels and blocks the flow of energies in the body. Patient feels lethargic, weak, stiff and tired with feverish feeling.

Stop These food items:

The best diet for Ulcerative colitis is stop consuming Milk, Cheese, Lassi (Butter milk). Although most of the texts in Ayurveda recommend Takra (buttermilk) But I have observed that approx.. 50 % of the patients do not respond well to Takra but 50 % of the patients respond well to Takra (buttermilk , Chachh or lassi) The butter milk should be prepared with Cumin seed water + Fennel Seed decoction or water. Then it has been found to be useful but usually I ask patients to stop consuming Chaach (Buttermilk / Lassi) and Milk and Cheese.

Sour fruits - Please don't consume Sour fruits like lemons, Citrus juices, oranges or mosami , kiwi and Pineapples. The tomatoes are also not recommended. Vinegar is really dangerous for ulcerative colitis patients.

Spices - Spices like Ginger, Garlic, Chillies are not recommended. Black pepper can be consumed in moderation with food. Bell Peppers can also be consume in moderation along with food.

Sweets - Sugar is not recommended at all.

Juices - All Packaged juices should be stopped immediately.

Fresh Juices - Pure pomegranate juice - 50 ml twice daily is recommended. Rather it is mandatory to be taken twice daily. Green grass juice - 10 ml once daily, Coriander leaves juice- 5 ml once daily or Marigold petals juice - 5 ml once daily is very good to stop bleeding. Rose petals juice 5 ml once daily is good. Just squeeze the petals in a clean cloth and take out the fresh juice. Apple juice / Beet juice can be consumed but only 20-30 ml twice daily.

Don't consume juices of other sour fruits. Don't consume too much fiber like Psyllium (Isabgol) If it suits you then it's ok, but in most of the cases, it doesn't suit. Do not consume too much fiber like Papaya or mangoes. Pears are ok like apples. Just consume pears only in moderation. Coconut water is perfect. Drink 1 fresh coconut water everyday in 2 divided doses.
