ABSTRACT
In today’s healthcare landscape, severe fever with delirium, restlessness, shivering, insomnia, and abnormal behavior are commonly seen in acute febrile conditions, especially when infection, postpartum weakness, or aggravated doshas are involved. Modern antipyretics may provide temporary relief, but repeated use can be associated with adverse effects and incomplete recovery. Ayurveda explains these states as vitiation of Vata, Pitta, and Kapha in fever disorders, leading to mental confusion, tremors, and systemic disturbance. Kesharadi Vati is a classical formulation designed for such acute febrile states. Through its hot, penetrating, and restorative ingredients, it helps calm agitation, reduce fever, and support recovery in delirious and severe fever conditions.
INTRODUCTION
Kesharadi Vati is a classical, fast-acting tablet formulation primarily indicated for severe fevers accompanied by acute mental agitation, delirium (Pralapak Sannipat), and postpartum febrile conditions (Sutikajwar). Named after its premium key ingredient, saffron (Keshar), this potent remedy swiftly targets critical clinical symptoms such as confusion, restlessness, insomnia, and abnormal behavioral disturbances. Clinically, it functions as a rapid antipyretic, potent nervine sedative, and emergency psychiatric-support formulation that grounds the mind and stabilizes the patient during severe, hyperactive febrile states.
CLASSICAL INDICATION
Fever associated with delirium and post-partum chronic fevers.

INGREDIENT DESCRIPTION
Keshar (Saffron)
Keshar (Saffron) provides a warm, stimulating, and restorative effect in severe fever states to support digestion, improve blood circulation, and aid recovery from the deep weakness associated with prolonged illness. Its primary active chemical constituents are crocin, crocetins, and safranal. Within the formula, crocin and crocetin function as powerful antioxidants and anti-inflammatory carotenoids that actively protect vulnerable brain tissues from the severe oxidative stress caused by hyperpyrexia (high fever). Concurrently, safranal acts as a central nervous system depressant and anticonvulsant, which directly helps to soothe acute mental agitation, quieten a chaotic mind, and alleviate the stubborn insomnia seen in delirious patients.
Jaiphal (Nutmeg)
Jaiphal (Nutmeg) is highly valued in acute conditions involving high fever, severe digestive disturbances, and extreme nervous hyperactivity, where it supports the appetite, calms Vata-Kapha disturbances, and relieves debilitating chills and body aches. It owes its therapeutic potency to key chemical constituents such as myristicin, elemicin, and safrole. In the context of Kesharadi Vati, myristicin and elemicin possess distinct sedative, muscle-relaxant, and anxiolytic (anxiety-reducing) properties that act directly on the central nervous system. This chemical action serves as a natural tranquilizer to quickly quieten a physically restless, shouting, or combative patient.
Javitri (Mace)
Javitri (Mace) is utilized for its strong digestive-enhancing and fever-relieving properties, effectively supporting the Agni (metabolic fire), clearing accumulated endotoxins (Ama), and resolving recurrent or lingering febrile states. Its primary chemical markers include macelignan and sabinene. Macelignan provides potent neuroprotective and anti-inflammatory benefits that help maintain blood-brain barrier integrity during periods of high systemic inflammation. Meanwhile, volatile monoterpenes like sabinene stimulate gastric enzymes and improve gut motility, ensuring that metabolic toxins driving the fever are broken down and eliminated.
Laung (Clove)
Laung (Clove) is a highly heating, sharp, and penetrating herb that helps reduce bodily heaviness, severe chills, and metabolic congestion, making it particularly useful when a high fever is accompanied by respiratory symptoms like cough, throat discomfort, or chest congestion. Its major active constituents are eugenol and beta-caryophyllene. Eugenol acts as a rapid and potent antipyretic by inhibiting the COX-2 enzyme, thereby blocking the synthesis of fever-inducing prostaglandins to lower body temperature. Simultaneously, beta-caryophyllene interacts with peripheral and central cannabinoid receptors to reduce neuroinflammation, rapidly calming behavioral distress and reducing pain.
Piplamool (Long Pepper Root)
Piplamool (Long Pepper Root) is a classical Jvaraghna (fever-destroying) counter-measure that strengthens overall digestion, reduces abdominal distension, and disrupts the repeating fever cycle related to underlying metabolic imbalances. Its therapeutic actions are driven by the alkaloids piperine and piperlongumine. Piperine acts as a powerful, scientifically proven bio-enhancer that modulates metabolic enzymes and increases the bioavailability and absorption of all other ingredients in the tablet—especially the heavy minerals. This ensures that the formulation enters the bloodstream rapidly to work as a swift emergency treatment, while piperlongumine provides additional systemic anti-inflammatory support.
Kasturi (Musk)
Kasturi (Musk) is traditionally described as an exceptionally potent, penetrating, and quick-acting substance used in critical, life-threatening fever states accompanied by severe delirium, extreme mental derangement, and acute obstruction of the body's channels. Its primary active chemical component is muscone, a volatile macrocyclic ketone. Because of its unique lipophilic chemical structure, muscone easily crosses the blood-brain barrier to act as a powerful central nervous system modulator and neuroprotective agent. It works almost instantly to clear profound mental confusion, arrest active delirium, and revive failing energy levels during a systemic crisis.
Rasmanikya (Purified Orpiment)
Rasmanikya is a classical Rasashastra (mineral preparation) deployed against stubborn, high-grade, and complex fevers because of its aggressive Kapha-Vata pacifying action and rapid effect when purely herbal alternatives fall short. Chemically, it consists of purified and structurally processed arsenic trisulfide (As2S3). Following traditional Ayurvedic purification (Shodhana), this crystalline compound acts as a powerful cellular metabolic stimulant that disrupts the replication cycle of deep-seated pathogens. It quickly clears stubborn systemic toxicity and breaks down the entrenched pathogenesis of the disease to drop high body temperatures rapidly.
Abhrak Bhasma (Mica Calx)
Abhrak Bhasma is an advanced mineral preparation that supports vital tissue strength, protects energy reserves (Ojas), and accelerates cellular recovery during prolonged or exhausting illness, preventing deep debility and protecting vital organs from hyperactive febrile stress. It consists of calcined mica, forming a complex nano-crystalline silicate of aluminum, magnesium, and iron embedded with essential trace elements. This bio-compatible mineral matrix acts as a potent immunomodulator and cellular rejuvenator that stabilizes cell membranes, prevents tissue wasting from extreme heat, and provides vital elemental support to maintain proper cardiac and neurological functions under intense physiological strain.
METHOD OF PREPARATION
All the ingredients are mixed and triturated in the juice of betel leaves and ginger juice for one day each. After proper processing, pills of half a Ratti (62.5 mg) each are prepared. The medicine is then stored properly for use in fever conditions.
