BRANCHIAL CLEFT CYST: OVERVIEW

Branchial cleft cyst is a rare birth defect which chiefly affects the neck. Though the defect is present at birth, it generally becomes evident in childhood or adolescence in most cases. An initial clinical presentation of the defect in adulthood is very rarely encountered.

Branchial cleft cyst (also called Branchial cleft remnant) is characterized by the appearance of a lump, which develops one or both sides of the affected child’s neck or underneath the collarbone. The occurrence of this congenital birth defect is linked to the ‘early embryonic development’ stage -- specifically the fifth week of fetal development -- during which the major neck structures are formed. Due to this defect, the normal development of the tissues in the neck and collarbone, or branchial cleft, is hampered.

The tissues comprising the neck structures formed during the ‘early embryonic development’ stage later become the cartilage, blood vessels, muscles and bone.

The appearance of Branchial cleft cyst is typically in the form of an opening, or cleft sinuses, on one or both sides of the child’s neck. Due to this abnormality, there is an atypical form of tissues within the neck. These tissues may lead to the formation of pockets called ‘cysts’ that contain fluid, and may form passages which drain to an opening in the skin surface called fistulas. The cysts, lined with skin and lymph cells, may become infected or cause skin irritation, and result in difficulties with breathing or swallowing. In some rare cases, cancerous tumors may develop at the site of a branchial cleft in adults.

There are four main types of Branchial cleft abnormalities --- (i) First branchial cleft anomalies, in which cysts develop around the earlobe or under the jaw (ii) Second branchial cleft sinuses, which open on the lower part of the neck (iii) Third branchial cleft sinuses, which develop close to the thyroid gland in the front part of the muscle which attaches to the collarbone (iv) Fourth branchial cleft sinuses, which affect the area below the neck. The most common type of Branchial cleft abnormality is ‘Second branchial cleft sinuses,’ or the failure of fusion of the second and third branchial arches.

Branchial cleft cysts account for nearly 20% of neck masses in children. While the cyst may generally not be dangerous, it is important to take the affected child to the doctor without any delays, for an official diagnosis of the problem.

SYMPTOMS OF BRANCHIAL CLEFT CYST

Branchial cleft cyst mostly does not cause any pain, except in cases when it gets infected. Some of the symptoms of a branchial cleft cyst include:

  • Pain in the affected area.
  • Feeling of pressure in the affected area.
  • Draining of fluid from the neck of the child.
  • Formation of a small lump or mass on the side of the neck close to the front edge of the sternocleidomastoid muscle.
  • Formation of a lump, dimple or skin tag on the child’s upper shoulder, or slightly underneath their collar bone.
  • Swelling or tenderness in the child’s neck, generally occurring with an upper respiratory infection.

Branchial cleft cysts generally develop in late childhood or early adulthood as a solitary, painless mass which becomes infected, though it is not noticed earlier.

Ayurvedic treatment of diseases begins with the identifying of the ‘body type’ of patients, for the purpose of finding out certain body tendencies which may trigger a sickness/disease. Dysferlinopathy comes under category of musculoskeletal diseases. It is an autosomal recessive neuromuscular disorder caused by deficiency of functional dysferlin protein due to mutations in dysferlin gene. This disease is characterized by progressive muscle wasting. The exact cause of this disease is not known as it is rare disease. Dysferlinopathy is most commonly diagnosed as Limb-girdle muscular dystrophy type 2B (LGMD2B) or Miyoshi muscular dystrophy 1.This disease occur in early adulthood between age of 16 and 25 that affects skeletal muscle of limbs and limb girdles.

As per Ayurveda for a healthy body three doshas- vata, pitta and kapha are required in balance state. When there is increase in vata dosha along with "Mamsa dhatu" (muscles) basic body structure, it leads to muscle destruction. These are cause of formation of Dysferlinopathy.

WHAT ARE THE TYPES OF DYSFERLINOPATHY?

Some patients have no symptoms, others feel severe functional disability. There are two types of Dysferlinopathy that shows various symptoms as per the condition:

  1. Miyoshi myopathy
  2. Limb-girdle muscular dystrophy type 2B

WHAT ARE THE SYMPTOMS OF DYSFERLINOPATHY?

As per two types of Dysferlinopathy there are different symptoms appear according to conditions. These symptoms are mentioned below:

1. Symptoms According To Miyoshi Myopathy

  • It causes wasting and weakness of muscles.
  • Early symptoms appear in calf muscles of legs.
  • Sometimes it is difficult for patients stand on tiptoe.
  • With progression of disease muscle weakness and atrophy spreads to the thighs, gluteal muscles, shoulder girdle muscles and forearm.
  • Few additional symptoms like difficulty in climbing stairs, walking, standing and decrease in grip strength.

2. Symptoms According To Limb-Girdle Muscular Dystrophy Type 2B:

  • The muscles in the shoulder and pelvic girdle are more affected.
  • Difficulty in running, climbing, walking and standing.
  • Sometimes patient feel difficulty in doing activities that require the arms to be raised like combing hair.

DIAGNOSIS OF DYSFERLINOPATHY

For diagnosis of Dysferlinopathy genetic diagnosis is important. Routine blood investigation, CK level, conventional needle electromyography, electrocardiography, 2D electrocardiogram should be performed.

NSCLC ALK DISEASE

Non-Small Cell Lung Cancer (NSCLC) starts when the cells of the lung become abnormal and begin to grow out of control. As more cells grow, they can form into a tumour and then spreads to the other parts of the body.

There are 2 types of non-small cell lung cancer:

  • Non-Small Lung Cancer (NSCLC): About 80-85% of the lung cancers consists of this type.
  • Small Cell Lung Cancer (SCLC): About 10-15% cancers consist of this type.

Lung cancers typically start in the cells lining the bronchi and other parts of the lung such as the bronchioles or alveoli.

WHAT ARE THE CAUSES OF NON-SMALL CELL LUNG CANCER?

The exact causes of Non-Small Lung Cancer are not known. But some risk factors for these cancers are known and how some of these cause cells to become cancerous.

  • The most common cause of NSCLC is tobacco smoking. Tobacco smoking is one of the most common causes of lung cancer. About 80% of lung cancer deaths are caused by smoking.

Azotemia is a condition when the kidneys are unable to get rid of enough nitrogen wastes and they get build up in the blood. The kidneys are damaged due to some disease or an injury and so they are not able to filter the blood properly.

Azotemia is defined as an increase in urea nitrogen and creatinine. It is a type of nephrotoxicity.

IT IS OF THREE TYPES

1. Prerenal Azotemia

Prerenal azotemia is the most common type of azotemia and can be reversed. This condition occurs when something that is affecting the blood circulation affects the kidney function and the fluid doesn’t flow enough through the kidneys. For example when there is low blood pressure to the kidney or due to irregular heart function, the kidney is unable to filter out the waste products or make urine. This leads to high levels of concentration of serum creatinine and urea. Other reasons that may cause reduced blood flow to the kidney are burns, dehydration, hemorrhage, long term vomiting or diarrhea, too much exposure to heat, shock or any blockage of the artery that supplies blood to the kidneys.

2. Intrarenal Azotemia

Intrarenal azotemia is also known as acute renal failure or acute kidney injury. It is attributed to the problem with the kidney itself like infections, sepsis or disease of kidney. The most common type of intrinsic azotemia or intrinsic renal failure is acute tubular necrosis. In this case, there is excess nitrogen in the blood due to side effects of certain medications like antibiotics, anti-fungal drugs, chemotherapy, bigological therapies etc. It can be due to diabetes also.

3. Postrenal Azotemia

Postrenal azotemia happens due to obstruction of urinary tract leading to renal failure. It can occur along with prerenal azotemia. It happens when there is a blockage in the urinary system like stones, tumors, infections, urinary catheter, urinary crystals or enlarged prostate gland.

Azotemia is common in people aged between 45 to 65 years.

Long term diarrhea, vomiting, bleeding can also lead to azotemia.

SYMPTOMS OF AZOTEMIA

  • Painful urination which can be infrequent and decreased in quantity
  • Discoloration or urine and pale skin
  • Muscle weakness and fatigue or loss of energy and fast pulse rate

DIET RECOMMENDED FOR AZOTEMIA

The patients are advised to increase vegetables and fiber in diet. Vegetables should be cooked properly with lot of water.

They need to avoid excess of potassium and magnesium and even monitor the protein intake. Foods rich in potassium are bananas, potatoes, tomatoes, beans, chocolate, nuts, peaches, oranges, milk, raisin etc. They need to be limited in diet. Too much protein also makes kidneys work harder so the intake of protein should be balanced.

Circumscribed or limited: few circle are seen on the trunk or limbs. It is limited to one or several patches. It is also called Plaque Morphea. This is the most common form with 3-4 oval lesions. The lesions are painless but might itch. The patches are 1 to 20 cm in diameter.

Generalized: Many circles are seen on the trunk or limbs. It is spread over larger areas of the body. It is more severe form of localized disease and might lead to disfigurement. The lesions may join together.

Linear: Lines of involvement on the limbs or head. It can be seen in early age of 20 years. It can affect not just the skin but underlying structures such as muscle and bone. It features a single band of thickened, discolored skin. If linear morphea occurs on the face, it might cause issues with eyes or alignment of teeth in school ages children.

Mixed: Combination of circumscribed and linear or generalized and linear.

Pansclerotic: Involvement of all of the skin. It is rapidly progressive type of morphea with many plaques. It spares hands and feet.

In this disorder, the skin changes appear on the abdomen, chest or back. It might happen on the face, arms or legs in some cases. It may also involve the inside of the mouth, genitals and the eyes. But it is mainly on the outermost layer of the skin. In rare cases, it may restrict the movement of the joints.

Morphea often occurs first in childhood or middle adulthood. It affects women more than men.

Symptoms of Morphea

Symptoms of Morphea depends on the type and stage/severity of the disorder. It is mainly asymptomatic, with occasional itching and pain.

The most visible symptom of Morphea is a reddish, purplish oval patch of skin. These patches gradually develop a lighter or whitish center. The skin then becomes hard, thickened, dry and shiny.

Loss of hair and sweat glands in the affected area is visible.

In children, the linear morphea on arms or legs can affect the underlying bone and interfere with bone growth. If on the head, it may extend inwards to the brain and causes headaches, nerve palsies or seizures.

If morphea crosses the joints, the thickening may limit joint movement.

Constriction of the chest wall leads to shortness of breath in case of pansclerotic morphea.

Oral and dental problems occur in case of teeth and jaw involvement in linear morphea. It leads to difficulty in chewing, jaw locking and pain.

Other symptoms include fatigue, muscle pains. Heartburn, eye dryness, cold hands etc.

Diagnosis of Morphea

Diagnoses of Morphea is done by doing various blood tests, skin biopsy and MRI.

A complete medical history and family health history is taken by the doctor.

Treatment of Morphea

Morphea may go away without treatment within 3 to 5 years but in some cases people are left with darkened areas of skin or muscle weakness.

In a bleeding disorder, the tendency of clotting blood naturally is affected. Liver, a largest internal organ of the body, is present on the upper right side of the abdomen. The removal of toxins from the body is the main function of this organ. Several health problems are there which are associated with liver. One of them is- fatty liver disease.

WHAT IS FATTY LIVER DISEASE?

Fatty liver disease, also known as Steatosis, occurs due to the buildup of fats in the largest internal organ. Generally, the presence of fats in the liver is regarded as normal, but once the fat composition reaches approx. 5 to 10 percent of the liver, then it is regarded as harmful fat.

Ayurvedic Treatment for Guillain-Barré Syndrome

What Is Guillain-Barre Syndrome (Gbs)?

It is also known as Acute Inflammatory Demyelinating Polyneuropathy.

It is a rare autoimmune disorder in which body's own immune system attacks on nerve cells of peripheral nervous system that results in weakness of muscles, numbness, loss of reflexes, tingling in arms, legs and other parts of body. In this case, insulation around myelin (nerves) is destroyed. Eventually it may results in paralysis and some long lasting effects on body.

It affects any age group but males are at high risk.

Types Of Gbs

  • Acute Inflammatory Demyelinating Poly Radiculoneuropathy(AIDP) - In This Case, Weakness Of Muscles Starts In The Lower Part Of The Body And Spreads Upwards. It Is More Common In USA.
  • Miller Fisher Syndrome (MFS) - Paralysis Starts With The Eyes, Associated With Unsteady Gait. It Is More Common In Asia.
  • Acute Motor Axonal Neuropathy (AMAN) And Acute Motor-Sensory Axonal Neuropathy (AMSAN)- It Is Frequent In China, Japan And Mexico.

What are the symptoms?

The first sign and common symptom is tingling sensation, numbness and pain. Other signs and symptoms may include:

  • Weakness
  • Severe back pain
  • Problem in talking, chewing and swallowing.
  • Loss of bladder control
  • High heart rate
  • Paralysis
  • Fluctuation in blood pressure
  • Problem in breathing

What Are The Causes?

The exact cause of GBS is unknown. This may be triggered by any infection that may arise after few days or weeks of respiratory or digestive tract infection.

Some infection that may be the cause of GBS are- Campylobacter jejuni cause diarrhea, Cytomegalovirus (CMV) causes fever, chills, sore throat, body aches, fatigue, Epstein-Barr virus (EBV) cause mononucleosis, Varicella-zoster virus cause chickenpox and shingles, Mycoplasma cause pneumonia and dengue fever.

In GBS, the body's immune system attacks body's nerves of peripheral nervous system. In this, the covering sheath (myelin sheath) is destroyed that results in loss of signals transmission in brain which leads to weakness, numbness or paralysis.

Diagnosis

In early stages, it is difficult to diagnose because the symptoms are similar to other neurological disorders. Then doctor may suggest some tests to confirm the disease which are given below:

  • Spinal tap / Cerebrospinal fluid analysis (CSF analysis) / lumbar puncture – It helps to check the protein level in CSF fluid which is elevated in GBS patients.
  • Nerve conduction studies
  • MRI of spinal cord
  • Blood tests for potassium level, sodium level which are elevated in GBS.
  • Electromyography
  • Test for anti ganglioside antibodies

Complications Associated With Gbs

If it is not treated with time then it may result in severe problems such as problem in breathing, heart problems like arrhythmia, fluctuation in blood pressure, problem in bowel and bladder, blood clots, relapse and death may due to heart problems.

Ayurvedic Outlook

As per Ayurveda, this condition is correlated with sarvanga gatavatavyadhi – vata disorder which is that is preceded by jvara.

Myelin sheath is a fatty in nature which is surrounded around the nerve cells that forms insulating layer which is necessary for proper functioning of nervous system. It is destroyed in case of GBS. It is compared to Medogata vata i.e. vitiation of vata. This destroys the myelin (fat) and affects the signal transmission. It may cause weakness of muscles and leads to GBS. It can be considered as Mamsagata vata.
