Treatment of Antisynthetase Syndrome in Ayurveda with Herbal Remedies

It might sound like a new term, but this malady has been bothering human civilization since the early centuries.

Syndrome, as the name suggests, is a combination of many signs and symptoms. Antisynthetase syndrome is a rare, chronic autoimmune disorder that affects the functionality of muscles and various other body parts. Antisynthetase syndrome is a rare idiopathic inflammatory condition which is presented by Arthralgia, Myositis, Mechanic hands, unexplained fever, Raynaud's phenomenon, Anti- ARS (serum antibodies to aminocyl transfer RNA synthetases and ILD (Interstitial lung disease). Antisynthetase syndrome

INTRODUCTION

Antisynthetase syndrome is also known by the names of AS syndrome or Anti-jo1 syndrome. As every patient is different from other and unique in itself so all clinical features will not be present in every patient but yes group them will be seen.

Antisynthetase syndrome is a rare disorder which affects more women as compared to men and usually the people above 50 years of age have this disease. Some patients present initially or only with Interstitial lung disease (ILD) and worsening of ILD can result in increased mortality rate.

SIGNS AND SYMPTOMS

  • Fever
  • Shortness of breath
  • Dysphagia
  • Cough
  • Weakness of proximal and axial muscles
  • Stiffness of muscles
  • Loss of appetite
  • Weight loss
  • Arthritis
  • Arthralgia
  • Esophageal dysmotility
  • Erthyema on neck, shoulder, chest and back
  • Pulmonary arterial hypertension

Clinical features in Antisynthetase syndrome may vary depending on time of appearance and severity. Main clinical features present are:

  • Fever- This usually occurs at the initial stages of the disease and present in only around 20-30% of the patients. There can be continuous or intermittent fever.
  • Inflammatory arthritis- Only the mild form is present in patients and around 50% or more patients suffer from this. Affects usually small joints of feet and hands and do not cause erosions of bones.
  • Myositis- This is one of the most common feature present in more than 90% of patients. Patients affected from this have anti jo-antibodies in their stream. Complaints are difficulty in ascending stairs, getting up from bed and pain in muscles is usually seen. Shortness of breath is caused due to weakness of respiratory muscles.
  • Mechanic’s hand- Only about 30% of patients get affected from this problem where thickening of skin occurs especially of finger tips. Name is given due to the appearance of hands like that of a mechanic.
  • Lung disease- Most of the patients of Antisynthetase syndrome are affected from ILD and suffers from gradual appearance of shortness of breath. Dry cough can also be present and eventually pulmonary hypertension also develops.
  • Raynaud’s phenomenon- 40% patients suffering from Antisynthetase syndrome develops Raynaud’s phenomenon. This is presented by reduction in blood supply to toes or fingers causing them to turn blue or red. Raynaud’s phenomenon is the response to sudden emotional stress or cold temperature.

CAUSES

Antisynthetase syndrome is a disorder of unknown origin. As immune system is found to be involved in its pathology hence is considered as a chronic autoimmune disease.

DIAGNOSIS

Presence of group of the clinical features discussed, points towards the Antisynthetase syndrome. However some investigations can confirm the diagnosis like Eclectromyography, MRI, Muscle biopsy, Lung function tests, CT scan, Lung biopsy and Muscle antibodies.

TREATMENT

In conservative mode of treatment Antisynthetase syndrome is treated depending upon the signs and symptoms present. Usually the Glucocorticosteroids are used in its treatment.
