Herbal Remedies for Solitary Rectal Ulcer Syndrome (SRUS)

Solitary rectal ulcer syndrome (SRUS) is an uncommon benign disorder of the rectum. As the name indicates, it is characterized with ulcer in rectum and it can be solitary or multiple. The size and extend of ulcers can vary from patient to patient. There are obvious symptoms, histological abnormalities and clinical findings. It is more often in females than males and in young adults, but it may occur to any age group too. The etiology is idiopathic. Ischemic pressure and local trauma by impacted stool can act as the contributing factors to SRUS.

CONTRIBUTING FACTORS TO SRUS

  • Chronic constipation (impacted stools)
  • Attempts to manually remove the impacted stools
  • Prolonged straining during bowel movements
  • Rectal prolapse
  • Pelvic floor muscle tightening which slows down the blood flow to the rectum
  • Intussusceptions (inversion of one part of intestine with other)

SIGNS & SYMPTOMS

Even though it shows obvious symptoms, for some patients it may remain asymptomatic. The hallmark sign of SRUS is the blood passage during defecation. The major symptoms include;

  • Rectal bleeding
  • Copious mucus discharge
  • Prolonged excessive straining
  • Perineal pain
  • Abdominal pain
  • Feeling of incomplete defecation
  • Feeling of fullness of pelvis
  • Fecal incontinence
  • Constipation (long standing)
  • Rectal prolapse (rarely)   

As it shows a range of symptoms which mimic inflammatory bowel syndrome, SRUS should be diagnosed with certain histo pathological features. These salient features include obliteration of lamina propria by fibrosis and abnormal smooth fiber extensions.

HERBAL REMEDIES FOR SRUS

In Ayurveda, the tridoshas – vata, pitta & kapha – vitiation is the prime cause for every disease. In SRUS disease, aggravation of tridosha predominantly pitta is the main factor. Accumulation of ama (due to sedentary life styles and unhealthy eating habits) along with pitta aggravation is the lone cause for SRUS. The aggravated ushna (hot) nature of pitta can trigger the formation of ulcers and can flare up the symptoms. The aggravated pitta in turn vitiates rakta dhatu and mamsa dhatu in the body leading to rectal bleeding and other associated symptoms.

Herbal Remedies for SRUS

1. Gandhak Rasayan

It is a potent anti inflammatory drug prepared out of shudh gandhak. It also has anti oxidant and adaptogenic properties. It helps to relieve any itching or burning sensation due to any underlying pathology. It helps to improve the skin health and is best for ulcer healing. It is the most effective medicine in Ayurveda for any kind of skin diseases. It rejuvenates the body and helps in prophylaxis. It also has anti microbial property.

Dosage – 2 capsules twice daily after meals with lukewarm water.

2. Neem Capsules

These capsules are prepared out of the standardized extract of neem (azadiracta indica). It improves the skin and hair health. It is very effective in almost all skin diseases including acne, eczema, psoriasis, microbial infections. It is also a potent anti inflammatory drug.

Dosage – 1 to 2 capsules twice a day after meals with lukewarm water.

3. Navkarshik Churna

It is a very effective blood purifier. It corrects the protein metabolism and cleanses the blood. It is used for pain management especially in arthritic conditions. As the name suggests, it contains 9 ingredients, each taken in karsha quantity (12 gms). The ingredients are amalaki (emblica officianalis), haritaki (terminalia chebula), vibhitaki (terminalia bellerica), neem (azadirachta indica), vacha (acorus calamus), manjishta (rubia cordifolia), katuki (piccorrhiza kurrao), guduci (tinospora cordifolia), daruharidra (berberis aristate). It balances the tridoshas.

Dosage – Half tsp daily after meals.

4. Curcumin Capsules

It is prepared out of turmeric (curcuma longa). It is a potent anti inflammatory drug. It helps to boost the immune system and also to modulate. They are mainly indicated to enhance the circulation in the body. Curcumin is a potent anti oxidant. Curcumin is very popular for its antimicrobial action. It helps to relieve the rashes and inflammation in viral infections.

Dosage – 1 capsule twice daily after meals with lukewarm water.

5. Giloy Capsules

Capsules are prepared out of guduci (tinospora cordifolia). It is considered to be as elixir and do possess rejuvenating properties. It also has anti allergic, anti inflammatory, anti bacterial and anti viral properties. It boosts the immune system and acts as immune modulator. It helps in the detoxification of the body, as it is enriched with a lot of natural antioxidants. It is a natural blood purifier.

Dosage – 1 capsule twice daily after meals with lukewarm water.

6. Nirgundi Oil

Nirgundi oil has anti inflammatory property and this property is very beneficial for SRUS patients. The ingredients of nirgundi oil include tila thaila (sesame oil), nirgundi (vitex negundo), turmeric (curcuma longa), manjishta (rubia cordifolia) and langali (gloriosa superb).

Dosage – For external local application only.

DIETARY & LIFESTYLE CHANGES FOR SRUS

  • Always stay hydrated. Have plenty of water
  • Increase the intake of dietary fibers. Have more fruits and vegetables. Dietary fiber supplementation is the best medicine for SRUS.   
  • Having whole fruits is better than juices
  • It is better to avoid staining for defecation
  • Clean and dry the anal area after each bowel movement. Avoid irritant soaps or bubble bath.
  • Sitz bath or hip baths can be made beneficial
